甘精胰岛素联合二甲双胍对糖尿病患者血糖水平及胰岛功能的影响

Effect of Insulin Glargine Combined with Metformin on Blood Glucose Level and Islet Function In Patients with Diabetes Mellitus

目的探究二甲双胍联合甘精胰岛素对糖尿病患者血糖水平及胰岛功能的影响。方法选取2017年9月至2018年9月于我院接受治疗的98例2型糖尿病患者为受试对象,按照入院顺序随机分为研究组与对照组各49例。对照组仅用甘精胰岛素进行治疗,研究组联合应用甘精胰岛素和二甲双胍。比较两组患者治疗前及治疗12周后血糖[空腹血糖(FPG)、餐后2h血糖(2hPG)、糖化血红蛋白(HbA1C)]及胰岛功能[胰岛素抵抗指数(HOMA-IR)、胰岛β细胞功能指数(HOMA-β)],比较两组患者治疗12周后炎性因子[高敏C反应蛋白(hs-CRP)]水平。结果治疗12周后,两组患者FPG、2hPG、HbA1C、HOMA-IR水平均较治疗前显著下降,研究组明显低于对照组;HOMA-β水平较治疗前显著上升,研究组明显高于对照组;治疗12周后,研究组hs-CRP水平明显低于对照组(P均<0.05)。结论甘精胰岛素联合二甲双胍能够更有效的控制糖尿病病情,降低血糖水平,促进胰岛功能恢复,疗效显著。

Objective To investigate the effect of insulin glargine combined with metformin on blood glucose level and islet function in patients with diabetes mellitus.Methods 98 patients with type 2 diabetes mellitus who were treated in our hospital from September 2017 to September 2018 were selected as subjects.According to the order of admission,they were randomly divided into study group and control group,49 cases in each group.Control group wasonly treated with insulin glargine,and study group was combined with insulin glargine and metformin.Blood glucose levels[fasting plasma glucose(FPG),postprandial 2h blood glucose(2hPG),glycated hemoglobin(HbA1C)]and islet function[insulin resistance index(HOMA-IR),islet β-cell function index(HOMA-β)]were compared between the two groups before treatment and after 12 weeks of treatment.The level of inflammatory factor[high-sensitivity C-reactive protein(hs-CRP)]was compared between the two groups after 12 weeks of treatment.Results After 12 weeks of treatment,the levels of FPG,2hPG,HbA1C and HOMA-IR in the two groups were significantly lower than those before treatment,and the levels in study group were significantly lower than those in control group.The HOMA-βlevel was significantly higher than that before treatment,and the level in study group was significantly higher than that in control group.After 12 weeks of treatment,the hs-CRP level in study group was significantly lower than that in control group(all P<0.05).Conclusion Insulin glargine combined with metformin can control the disease more effectively,lower blood glucose level and promote the recovery of islet function,with significant efficacy.
