老年髋关节置换术患者应用输氧联合呼吸功能训练干预对其术后谵妄发生情况及睡眠质量的影响

Effect of Oxygen Therapy Combined with Respiratory Function Training on Incidence of Postoperative Delirium and Sleep Quality of Elderly Patients Undergoing Total Hip Arthroplasty

目的观察老年髋关节置换术(total hip arthroplasty,THA)患者联用输氧及呼吸功能训练干预对其术后谵妄(postoperative delirium,POD)发生率及睡眠质量的影响。方法选择本院2019年4月至2020年4月进行初次单侧THA的老年病人308例为研究对象,通过随机数表法将患者分为观察组和对照组,每组154例。观察组予以呼吸功能训练联合输氧治疗,对照组仅进行输氧治疗,采用谵妄评定量表中文修订版(CAM-CR)以及匹兹堡睡眠质量指数量表(PSQI)对比两组治疗5d内的POD发生情况和睡眠质量的变化状况。结果术后5d内,观察组POD总发生率少于对照组(P<0.05),同时观察组治疗5d后睡眠质量评分低于对照组(P<0.05)。结论对老年THA患者予以输氧联合呼吸功能训练干预可以减少患者POD发生情况,同时可以显著提高患者的睡眠质量,值得推广。

Objective To observe the effect of oxygen therapy combined with respiratory function training on the incidence rate of postoperative delirium(POD)and sleep quality of elderly patients undergoing total hip arthroplasty(THA).Methods The study subjects were 308 elderly patients who had underwent the first unilateral THA in the hospital between April 2019 and April 2020.They were divided into observation group and control group by random number table method,154 cases in each group.The observation group was given respiratory function training combined with oxygen therapy,and the control group was given oxygen therapy only.The Confusion Assessment Method-Chinese Revision(CAM-CR)and Pittsburgh Sleep Quality Index(PSQI)were used to compare the incidence of POD and changes in sleep quality between the two groups within 5 days of treatment.Results The total incidence of POD in the observation group was lower than that in the control group within 5 days after surgery(P<0.05),and the sleep quality score of observation group was lower than that of the control group after 5 days of treatment(P<0.05).Conclusion Applying oxygen therapy combined with respiratory function training in elderly patients undergoing THA can reduce the incidence of POD,and significantly improve their sleep quality.
