经颅多普勒超声对Alzheimer病和血管性痴呆的诊断价值分析

The Diagnostic Value of Transcranial Doppler Ultrasound in Alzheimer’s Disease and Vascular Dementia

目的:探析经颅多普勒超声诊断(TCD)Alzheimer病(AD)和血管性痴呆(VD)的诊断效果及价值。方法:选取本院于2017年1月~2019年1月收治的30例AD患者、25例VD患者及30例健康体检者,均行TCD检测,对比分析三组患者的MFV水平及V_(MCA)水平与MMSE的相关性。结果:AD组TCD异常率为92.38%,VD组TCD异常率为95.47%;AD与VD组患者血流速度均低于对照组(P<0.05),AD组与VD组差异不明显(P>0.05);血流速度持续性降低,精神心理状况越差,呈正性相关性。结论:TCD可通过检测MFV水平变化及痴呆严重程度,为鉴别诊断VD、AD患者提供实质性的参考价值。

Objective:To explore the diagnostic effect and value of TCD in Alzheimer’s disease(AD)and vascular dementia(VD).Methods:30 AD patients,25 VD patients and 30 healthy physical examination patients admitted to our hospital from January 2017 to January 2019 were selected and all underwent TCD test.MFV,PI levels and the association between MFV and MMSE in the three groups were compared and analyzed.Results:The abnormal rate of TCD in AD group was 92.38%,and that in V D group was 95.47%.The blood flow velocity of AD and VD group was lower than that of control group(P<0.05),and the difference between AD group and VD group was not significant(P>0.05).The lower the blood flow velocity,the worse the mental state,the positive correlation.Conclusion:TCD can provide substantial reference value for differential diagnosis of VD and AD by detecting changes in MFV level and severity of dementia.
