腔内微波闭合术治疗大隐静脉曲张的疗效观察

The observation of therapeutic efficacy on endovenous microwave ablation in the treatment of varicosis great saphenous vein

目的探讨腔内微波闭合术(EMA)治疗大隐静脉曲张的临床疗效。方法选取2018年1月-2019年1月天津市第一中心医院收治的197例(231条肢体)大隐静脉曲张患者为研究对象,随机分为对照组100例和观察组97例。对照组采取大隐静脉高位结扎剥脱术,观察组采取EMA。比较分析两组患者手术时间、术中失血量、术后住院时间、术后48 h VAS评分、术后并发症发生率及术前、术后的VCSS评分、AVVQ评分情况。结果观察组手术时间、术中失血量、术后住院时间及术后48 hVAS疼痛评分均优于对照组,差异均有统计学意义(P<0.05)。观察组术后并发症发生率为9.3%,明显低于对照组的28.0%,差异有统计学意义(P<0.05)。两组术后1年VCSS评分明显低于手术前,差异均有统计学意义(P<0.05)。术后3个月,观察组AVVQ评分低于对照组,差异有统计学意义(P<0.05)。结论EMA是治疗大隐静脉曲张的有效方法,值得临床推广应用。

Objective To investigate the clinical efficacy of endovenous microwave ablation(EMA)in the treatment of varicosis great saphenous vein.Methods From January 2018 to January 2019,197 cases(231 limbs in total)with varicosis great saphenous vein treated in Tianjin First Central Hospital were selected and randomly divided into control group with 100 cases and observation group with 97 cases.The control group was treated with high ligation and stripping of great saphenous vein,and the observation group was treated with EMA.The operation time,intraoperative blood loss,postoperative hospital stay,48 h VAS score,the incidence of postoperative complications,preoperative and postoperative VCSS score,AVVQ score of the two groups were compared and analyzed.Results The operation time,intraoperative blood loss,postoperative hospital stay and 48 h VAS pain score of the observation group were better than those of the control group,and the differences were statistically significant(P<0.05).The incidence of postoperative complications in the observation group was 9.3%,which was significantly lower than 28.0%of the control group,and the difference was statistically significant(P<0.05).The VCSS score of the two groups was significantly lower than those before operation,and the differences were statistically significant(P<0.05).Three months after operation,the AVVQ score of the observation group was lower than that of the control group,and the difference was statistically significant(P<0.05).Conclusion EMA was a effective way treating varicosis of great saphenous vein,which was worthy of clinical promotion and application.
