高强度间歇训练对冠心病患者心肺功能、生活质量及miRNA表达谱的影响 被引量:9

Effects of high intensity intermittent training on cardiopulmonary function,quality of life and expression profile of microRNAs in patients with coronary heart disease

摘要 目的研究高强度间歇训练对冠状动脉粥样硬化性心脏病(冠心病)患者的心肺功能及生活质量的影响并筛选鉴定与高强度间歇训练运动调控相关的差异化表达miRNA。方法选取2018年1月~12月于郑州中心医院进行规范治疗的冠心病患者80例为研究对象,随机分为对照组与高强度间歇训练组。对照组采用常规药物治疗,训练组则进一步联合高强度间歇训练,8周后再次评估两组患者的心功能、肺功能及生活质量变化,同时采用AffymetrixGeneChip miRNA芯片检测两组患者血清表达差异的miRNA并采用Realtime PCR进行验证。结果在治疗结束后发现,对照组和HIIT组患者6MWD、LVEF、LVEDD较治疗前均得到显著改善(P<0.05);而HIIT组患者的心功能改善显著优于对照组(P<0.05);同样治疗结束后HIIT组患者AT时氧脉搏、AT时分钟通气量、最高氧脉搏和最高每分钟通气量指标显著优于治疗前(P<0.05)和对照组患者(P<0.05);生活质量方面,经过8周干预治疗后,对照组和HIIT组患者生理功能、生理职能、躯体疼痛和总体健康较治疗前均得到显著改善(P<0.05);而HIIT组患者的生活质量改善显著优于Control组患者(P<0.05)。进一步两组患者的miRNA基因芯片筛选发现训练组患者血清miR-20a-5p,miR-93-5p和miR-1287-5p表达显著增加,而miR-7706,miR-28-5p和miR-125b-5p表达显著降低,可能参与了高强度间歇训练改善冠心病的进程。结论高强度间歇训练对于冠心病患者的心肺功能及生活质量都能够起到显著改善作用,其获益机理可能与调控miRNA差异化表达有关。 Objective To analyze the effect of high intensity intermittent training on cardiopulmonary function and quality of life in patients with coronary heart disease,to screen and identify differentially expressed microRNAs.Methods 80 patients with coronary heart disease treated in our hospital from January 2018 to December 2019 were randomly divided into control group and high intensity intermittent training group.The control group was treated with routine drugs,while the training group was further treated with high intensity intermittent training.After 8 weeks,the changes of cardiac function,lung function and quality of life were analyzed.At the same time,AffymetrixGeneChip microarray was used to detect the expression of microRNAs between the two groups,which was verified by Realtime PCR.Results After treatment,it was found that 6MWD,LVEF and LVEDD of the control group and HIIT group were significantly improved compared with those before treatment(P<0.05).The improvement of cardiac function in HIIT group was significantly better than that in control group(P<0.05).Similarly,AT oxygen pulse rate,AT minute ventilation rate,maximum oxygen pulse rate and maximum ventilation rate per minute in HIIT group were significantly better than those before treatment(P<0.05)and control group(P<0.05).About quality of life,after 8 weeks of intervention,the physiological function,physiological function,physical pain and general health of the patients in the control group and the HIIT group were significantly improved(P<0.05).But the quality of life of the HIIT group was significantly better than that of the Control group(P<0.05).The expression of serum miR-20a-5p,miR-93-5p and miR-1287-5p was significantly increased in the training group,while the expression of miR-7706,miR-28-5p and miR-125b-5p was significantly decreased,which may be involved in the process of improving coronary heart disease by high intensity intermittent training.Conclusion High intensity intermittent training can significantly improve cardiac function,lung function and quality of life in patients with coronary heart disease,which may be related to the regulation of differentiated expression of miRNAs.
