224例实证功能性便秘患者舌象特征参数分析

Analysis of the Tongue Characteristic Parameter of 224 Functional Constipation Patients of Excessive Syndrome

目的探讨不同证型实证功能性便秘患者的舌象特征差异。方法收集224例实证功能性便秘患者,其中热积秘90例、寒积秘68例、气滞秘66例。采用舌面一体仪记录分析224例患者的舌象特征数据,采用HSV模型记录舌质和舌苔色度,并观察舌质(裂纹、点刺、齿痕、胖瘦)指数和苔质(厚薄、润燥、腐腻、剥脱)指数,比较不同证型的舌象差异。结果热积秘以红舌、薄黄苔为多;寒积秘多为淡白胖舌、薄白苔,且多见齿痕;气滞秘以淡红舌、薄白苔为多见。舌象特征数据示,寒积秘舌质色度明度值最大、饱和度值最小,舌苔色度明度值、齿痕指数最大(P<0.05或P<0.01)。热积秘的裂纹指数、剥脱指数最大(P<0.05或P<0.01)。结论实证功能性便秘中,寒积秘的舌色、苔色最浅最偏白,也更易出现齿痕舌,热积秘则多见裂纹舌、剥脱苔。

Objective To explore the differences of tongue characteristics in patients of functional constipation of different excessive syndromes.Methods Totally 224 cases were collected with 90 cases of heat accumulation,68 cases of cold accumulation and 66 cases of qi stagnation.Tongue-surface integrated instrument was used to record and analyze the tongue feature data.The HSV model was used to record tongue texture(cracks,pricks,tooth marks,fat or thin)index and tongue coating texture(thickness,moisturizing,greasy,exfoliating)index to compare the differences in tongue picture of different syndrome types.Results Red tongue and thin yellow coating were more common in heat accumulation constipation;cold accumulation constipation were mostly pale and fat tongue,thin white coating,and tooth marks were more common;qi stagnation constipation was more common in pale red tongue and thin white coating.Tongue feature data showed that the cold accumulation constipation had the largest chroma and lightness value,the smallest saturation value,and the tongue coating chroma and lightness value and tooth mark index were the largest(P<0.05 or P<0.01).The crack index and exfoliation index of heat accumulation were the largest(P<0.05 or P<0.01).Conclusion In excessive type functional constipation,white tongue and tongue coating are common in cold accumulation constipation,and it is more prone to have tooth marks tongue.In heat constipation,cracked tongue and exfoliation tongue coating are more common.
