膝关节镜清理联合几丁糖注射治疗膝关节骨性关节炎的临床效果

Clinical effect of arthroscopic debridement plus medical chitosan in knee osteoarthritis

目的研究几丁糖对膝关节骨性关节炎患者的治疗效果。方法选取2016年5月~2019年1月广州市荔湾区人民医院收治的172例膝关节骨性关节炎患者,按随机数表法分为4组,其中43例患者单纯采用膝关节镜清理作为关节镜组;43例患者采用单纯几丁糖注射治疗作为几丁糖组;43例患者采用膝关节镜联合生理盐水注射作为对照组;最后43例患者采用膝关节镜联合几丁糖治疗作为观察组。结果观察组治疗总有效率明显高于对照组、膝关节镜组和几丁糖组,差异均有统计学意义(P<0.05)。治疗前4组患者膝关节功能评分和VAS评分对比差异无统计学意义(P>0.05);治疗1个月、6个月以及12个月后观察组明显低于其他3组,差异有统计学意义(P<0.05)。4组患者并发症包括膝关节局部感染、膝关节神经损伤以及膝关节韧带损伤,4组并发症发生率对比差异无统计学意义(P>0.05)。结论关节镜清理联合几丁糖注射关节腔对膝关节骨性关节炎患者具有较好疗效,可减轻患者痛苦,改善关节功能,安全性高。

Objective To study the therapeutic effect of medical chitosan in knee osteoarthritis(KOA).Methods 86 KOA patients treated from May 2016 to January 2019 in our hospital were selected and randomly assigned to two groups,43 cases in each group.The control group was treated with arthroscopic debridement.The observation group was treated with arthroscopic debridement plus medical chitosan.Results The total effective rate in the observation group(95.35%)was significantly higher than that of control group(81.40%)(P<0.05);after one month,six months and twelve months,knee joint function scores and VAS(visual analogue scale)scores in the observation group were better than those of control group(P<0.05);the incidences of complications like infection,nerve injury and ligamentous injury between groups were not significantly different(P>0.05).Conclusion The combined mode of arthroscopic debridement and medical chitosan can increase the therapeutic effect,relieve the pain and improve the knee joint function and treatment safety.It is worthy of clinical promotion and application.
