量化定位角度牵引治疗神经根型颈椎病30例临床分析

Clinical analysis of 30 cases of cervical spondylotic radiculopathy treated by quantitative positioning angle traction

目的:观察量化定位角度牵引联合针灸及中药封包治疗神经根型颈椎病(CSR)的临床疗效。方法:选取2019年5月至2020年5月陕西中医药大学附属医院收治的60例CSR患者,按随机数字表法分为观察组和对照组,每组30例。对照组采取中立位0度角牵引联合针灸及中药封包,观察组采取量化定位角度牵引(结合患者颈椎X线片,少数患者需要结合CT或MRI选择牵引角度)联合针灸及中药封包。治疗2周后比较两组患者的临床疗效、疼痛视觉模拟评分(VAS)、颈椎功能障碍指数量表(NDI)评分、Borden法测量的颈椎曲度D值及不良反应的差异。结果:观察组总有效率为96.6%,对照组总有效率为83.3%,观察组疗效优于对照组(P<0.05)。两组在治疗2周后疼痛VAS评分、颈椎曲度D值及NDI评分明显降低(P<0.01),且观察组改善情况均优于对照组(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:量化定位角度牵引联合针灸及中药封包治疗CSR患者临床疗效确切,可明显降低VAS评分、NDI评分及颈椎曲度D值,提高临床治疗疗效和患者的生存质量,值得临床推广使用。

Objective:To observe the clinical efficacy of quantitative positioning angle traction combined with acupuncture and TCM packet in treatng cervical spondylotic radiculopathy(CSR).Methods:A total of 60 patients with CSR treated in The affiliated Hospital of Shanxi University of Chinese Medicine from May 2019 to May 2020 were randomly divided into observation group and control group,30 cases in each group.The control group was treated with neutral position zero degree angle traction combined with acupuncture and TCM packet,while the observation group was treated with quantitative positioning angle traction(combined with cervical Xray,some traction angles were chosen by combining with CT or MRI in a few cases)combined with acupuncture and TCM packet.After 2 weeks of treatment,the clinical efficacy,visual analogue score(VAS),neck disability index(NDI),cervical curvature D value measured by Borden method and adverse reactions were compared between the two groups.Results:The total effective rate of the observation group was 96.6%,while that of the control group was 83.3%.The clinical efficacy of the observation group was better than that of the control group(P<0.05).After 2 weeks of treatment,the VAS score,cervical curvature D value and NDI score in the two groups decreased significantly(P<0.01),and the improvement in the observation group was better than that in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Quantitative positioning angle traction combined with acupuncture and TCM packet is effective in treating CSR patients,which can significantly reduce VAS score,NDI score and cervical curvature D value,improve clinical efficacy and patients’life quality,which is worthy applying.
