2006—2015年南宁市某医院肝癌患者住院费用趋势分析

Trend analysis of inpatient costs of liver cancer patients in a hospital in Nanning,2006-2015

目的:探讨南宁市某医院肝癌患者住院费用的变化趋势,为减轻肝癌住院患者直接经济负担提供科学依据。方法:收集2006—2015年南宁市某医院1117例肝癌住院患者病例资料,采用中位数、构成比、环比增长率、结构变动贡献率等指标对住院费用进行统计描述,分析其结构和变化趋势。结果:在剔除物价因素影响后,2006—2015年肝癌患者例均、次均和日均住院费用分别增长了1.7倍、1.3倍和2.6倍;住院费用在10年间先大幅升高后明显降低再逐年回升,但2012年后增长的势头较2009年以前放缓;药品费和诊查费在住院费用构成中占据第1位和第2位,占住院费用总额超半数;药品费、诊查费、其他费用和治疗费对住院费用结构变动的贡献最大。结论:肝癌患者住院费用较高,增长较快,除控制药占比外,还需其他措施控制总住院费用的增长。

Objective:To explore the trend of inpatient costs of liver cancer patients in a hospital in Nanning,and to provide a scientific basis for reducing the direct financial burden of liver cancer inpatients.Methods:The data of 1,117 cases of liver cancer patients hospitalized in a hospital in Nanning from 2006 to 2015 were collected to statistically describe the inpatient costs in terms of median,composition,ring growth rate and structural change contribution,and to analyze their structure and trend.Results:After excluding the influence of inflation,the per capita,secondary per capita and average daily inpatient costs of liver cancer patients increased 1.7 times,1.3 times and 2.6 times respectively from 2006 to 2015;in the past 10 years,inpatient costs rose sharply,then decreased significantly and then rose again year by year,but the momentum of increase slowed down after 2012 compared with that before 2009;drug costs and consultation fees occupy the 1st and 2nd place in the composition of inpatient costs and accounts for more than half of the total inpatient costs;drug costs,consultation fees,other costs and treatment fees contribute most to the changes in the structure of inpatient costs.Conclusion:Inpatient costs for patients with liver cancer are high and growing rapidly,and other measures are needed to check the growth of total inpatient costs in addition to controlling the proportion of drug sales in the total income of a hospital.
