PMS2、MLH1、MSH2、MSH6在散发性结直肠癌中错配修复蛋白的表达及临床意义 被引量:2

摘要 目的探讨PMS2、MLH1、MSH2、MSH6等错配修复蛋白在散发性结直肠癌患者中的表达水平及临床意义。方法选取84例散发性结直肠癌患者,采用免疫组织化学检测法检测其组织样本中4种错配修复蛋白的阳性表达,分析其与结直肠癌患者临床特征的关系。结果直肠散发性结直肠癌和淋巴转移患者的PMS2阳性表达率高于左半结肠、右半结肠和未淋巴转移患者,差异均有统计学意义(χ^2分别=6.22、5.74、5.39,P均<0.05);直肠、中低分化和淋巴转移的散发性结直肠癌患者的MLH1阳性表达率高于左半结肠、右半结肠、高分化和未淋巴转移患者,差异均有统计学意义(χ^2分别=2.44、7.60、3.86、5.39,P均<0.05);淋巴转移的散发性结直肠癌患者的MSH2阳性表达率高于未淋巴转移者,差异有统计学意义(χ^2=4.37,P<0.05)。Spearman相关分析显示,MSH2阳性表达与MSH6阳性表达有相关性(rs=0.67,P<0.05),PMS2阳性表达与MLH1阳性表达有相关性(rs=0.81,P<0.05)。结论淋巴转移、病灶部位分化程度是影响PMS2与MLH1、MSH2阳性表达的影响因素;MSH2阳性表达与MSH6阳性表达有相关性,PMS2阳性表达与MLH1阳性表达有相关性。 Objective To investigate the expression level and clinical significance of mismatch repair proteins such as PMS2,MLH1,MSH2,MSH6 in patients with sporadic colorectal cancer.Methods A total of 84 patients with sporadic colorectal cancer were selected.Immunohistochemistry was used to detect the positive expression of four mismatch repair proteins in their tissue samples,and the relationship between them and the clinical characteristics of colorectal cancer patients was analyzed.Results The positive expression rates of PMS2 in patients with sporadic rectal colorectal cancer and lymphatic metastasis were higher than that of the left colon,right colon and non-lymphatic metastasis(χ^2=6.22,5.74,5.39,P<0.05).The positive expression rates of MLH1 in patients with sporadic colorectal cancer with moderate,poorly differentiated,and lymphatic metastasis were higher than that of left colon,right colon,well differentiated and non-lym⁃phatic metastasis(χ^2=2.44,7.60,3.86,5.39,P<0.05).The positive expression rate of MSH2 in sporadic colorectal cancer patients with lymphatic metastasis was higher than that of patients without lymphatic metastasis(χ^2=4.37,P<0.05).Spearman correlation analysis showed that the positive expression of MSH2 was correlated with the positive expression of MSH6(rs=0.67,P<0.05),and the positive expression of PMS2 was correlated with the positive expression of MLH1(rs=0.81,P<0.05).Conclusion Lymphatic metastasis,location and degree of differentiation are influencing fa⁃ctors for the positive expression of PMS2 and MLH1 and MSH2;the positive expression of MSH2 is correlated with the positive expression of MSH6,and the positive expression of PMS2 is correlated with the positive expression of MLH1.
