活血定眩汤配合针灸治疗椎动脉型颈椎病的临床疗效观察

Clinical Observation of Huoxue Dingxuan Decoction Combined with Acupuncture in Treating Cervical Spondylosis of Vertebral Artery Type

目的:探讨活血定眩汤配合针刺疗法治疗椎动脉型颈椎病的效果。方法:将104例符合纳入标准的椎动脉型颈椎病患者随机分为两组,治疗组予以活血定眩汤联合针灸治疗,对照组予以口服氟桂利嗪胶囊配合针灸治疗。观察比两组患者治疗后的临床疗效、经颅多普勒血流参数以及复发率。结果:1)治疗前两组患者左、右椎动脉及基底动脉平均血流速度差异不明显,无统计学意义(P>0.05);治疗后两组患者动脉血流速度均提高,且治疗组较对照组流速升高更明显(tL=5.647,P<0.05;tR=6.124,P<0.05;tS=5.133,P<0.05);2)治疗组治疗后的总体疗效明显优于对照组。[治疗组R 1=0.432,95%CL为(0.364,0.501);对照组R 2=0.567,95%CL为(0.490,0.643);t=2.583,P<0.05)];3)治疗半年后随访,治疗组的复发率及不良反应发生率明显低于对照组(χ^2=5.626,P<0.05)。结论:活血定眩汤配合针灸在椎动脉型颈椎病的临床疗效及治疗周期都显著优于氟桂利嗪,而不良反应发生率及复发率均明显低于对照组,可见活血定眩汤对眩晕患者有积极地治疗作用,值得在临床中推广运用。

Objective:To explore the effect of Huoxue Dingxuan Decoction combined with acupuncture in the treatment of cervical spondylosis of vertebral artery type.Methods:104 patients with cervical spondylosis of vertebral artery type were randomly divided into the control group and the treatment group.The treatment group was treated with Huoxue Dingxuan Decoction combined with acupuncture,and the control group was treated with Flunarizine Capsule combined with acupuncture.The clinical efficacy,transcranial Doppler blood flow parameters and disease recurrence rate were observed and compared between the two groups after the treatment.Results:There was no statistical difference in the mean blood flow velocity of left and right vertebral artery and basilar artery between the two groups before the treatment(P>0.05);the arterial blood flow velocity increased in both groups after the treatment,of which the increases in the treatment group were more significant(tL=5.647,P<0.05;tR=6.124,P<0.05;t S=5.133,P<0.05).The overall curative effect of the treatment group was significantly higher than that of the control group[R1=0.432,95%CL(0.364,0.501);R2=0.567,95%CL(0.490,0.643);t=2.583,P<0.05)].During the follow-up of half a year after the treatment,the recurrence rate and the incidence of adverse reactions in the treatment group were significantly lower than those in the control group(χ^2=5.626,P<0.05).Conclusion:The clinical efficacy and treatment cycle of Huoxue Dingxuan Decoction combined with acupuncture are significantly better than those of Flunarizine combined with acupuncture in the treatment of cervical spondylosis of vertebral artery type.The therapy has lower incidence of adverse reactions and recurrence rate,which is worth popularizing in clinic.
