高尿酸血症患者尿酸水平与糖耐量异常的相关性研究

Correlation study on level of uric acid and the abnormal glucose tolerance in hyperuricemia patients

目的探讨高尿酸血症患者尿酸水平和糖耐量异常的相关性。方法选择济南市人民医院收治的高尿酸血症患者100例为观察组,并根据血糖水平将其分为血糖正常组(A组)、糖耐量异常组(B组)、空腹血糖受损组(C组)3个亚组;选择同期来济南市人民医院健康体检者100人为对照组。抽取所有受试者清晨空腹静脉血进行血尿素及糖耐量相关指标检测。结果观察组患者血清尿酸、空腹血糖、餐后2 h血糖、空腹胰岛素、餐后2 h胰岛素、胰岛素抵抗-指数高于对照组,而胰岛敏感指数低于对照组(P<0.01)。A、B、C 3亚组间血清尿酸和糖耐量水平之间差异有统计学意义(P<0.01),其中C组的血清尿酸和糖耐量相关指标显著高于A、B两组,且B组也高于A组,差异有统计学意义(P<0.05);相关性研究表明,血清尿酸与空腹血糖、餐后2 h血糖、空腹胰岛素、餐后2 h胰岛素、胰岛素抵抗-指数均呈正相关(P<0.01),而与胰岛敏感指数呈负相关(P<0.01)。结论高浓度的血尿酸水平会引发糖耐量异常,降低胰岛素敏感性,提高胰岛素抵抗,需要临床加以关注。

Objective To explore the correlation between the level of uric acid and the abnormal glucose tolerance in hyperuricemia patients.Methods A total of 100 patients with hyperuricemia were selected as the observation group in Jinan city people’s hospital,and according to the blood glucose level,they were divided into 3 subgroups:normal blood glucose group(group A),abnormal glucose tolerance group(group B),and impaired fasting blood glucose group(Group C).100 healthy subjects who came to the hospital at the same time were selected as the control group.Blood urea and glucose tolerance were measured in fasting venous blood of all subjects in the morning.Results SUA,FPG,2 hPG,fins,pins and homa-or were significantly higher in the observation group than those in the control group,while ISI was lower in the observation group(P<0.01).In addition,there were significant differences in the levels of SUA and glucose tolerance among the 3 subgroups(P<0.01),among which,the related indexes of SUA and glucose tolerance in group C were significantly higher than those in group A and B,and those in group B were also higher than those in group A(P<0.05).The correlation study showed that SUA was positively correlated with FPB,2 hgp,fins,pins and HOMA-OR(P<0.01),but negatively correlated with ISI(P<0.01).Conclusion High concentration of serum uric acid can lead to abnormal glucose tolerance,decrease insulin sensitivity and improve insulin resistance,which needs clinical attention.
