艾司西酞普兰联合利培酮治疗精神分裂症阴性症状的疗效

Efficacy of esitalopram combined with risperidone in the treatment of negative symptoms of schizophrenia

目的:探讨艾司西酞普兰联合利培酮治疗精神分裂症阴性症状的疗效及对社会功能的影响。方法:选择2017年10月至2019年6月在普陀区精神卫生中心门诊或住院的精神分裂症患者为研究对象,将其随机分为研究组和对照组。研究组44例患者采用艾司西酞普兰联合利培酮治疗;对照组35例患者采用单一抗精神病药物利培酮治疗。观察12周,两组分别于治疗前、治疗10天、4周、8周、12周采用阳性与阴性症状量表(PANSS)及阴性症状评估量表(SANS)评定疗效;于治疗12周前后,采用个人和社会功能量表(PSP)评定社会功能。结果:两组PANSS、SANS、PSP评分治疗后均较治疗前明显改善(P<0.05)。两组比较,PANSS、SANS评分在治疗前及治疗10天时差异无统计学意义(P>0.05);在治疗4周、治疗8周及治疗12周时差异有统计学意义(t=-2.246,-2.209,-2.384,-2.121,-2.603,-3.368;P<0.05)。两组比较,PSP评分在治疗前差异无统计学意义(P>0.05);在治疗后差异有统计学意义(t=3.158,P<0.01)。结论:艾司西酞普兰联合利培酮治疗与利培酮单一治疗对精神分裂症阴性症状及社会功能均有改善,但联合治疗疗效更优。

Objective:To explore the efficacy of escitalopram combined with risperidone in the treatment of negative symptoms of schizophrenia and its impact on social function.Methods:From January 2019to December 2019,patients with schizophrenia who were in the outpatient or inpatient department of Putuo mental health center were randomly divided into study group and control group.44patients in the study group were treated with escitalopram and risperidone,and 35patients in the control group were treated with risperidone.After 12weeks of treatment,the positive and negative symptom scale(PANSS)and negative symptom assessment scale(SANS)were used to evaluate the efficacy of the two groups before treatment,10days,4weeks,8weeks and 12weeks of treatment,and the individual and social function scale(PSP)was used to evaluate the social function before and after 12weeks of treatment.Results:The scores of PANSS,sans and PSP in the two groups were significantly improved after treatment(P<0.05).There were no significant differences in PANSS and sans scores between two groups before treatment and 10days after treatment(P>0.05);There were significant differences in 4weeks,8weeks and 12weeks after treatment(t=-2.246,-2.209,-2.384,-2.121,-2.603,-3.368,P<0.05).Compared with two groups,there was no significant difference in PSP score before treatment(P>0.05),and there was significant difference after treatment(t=3.158,P<0.01).Conclusion:Escitalopram combined with risperidone and risperidone alone can improve the negative symptoms and social functions of schizophrenia,but the combined treatment is more effective.
