系统性红斑狼疮患者中性粒细胞亚群与冠心病病情的相关性:倾向性评分匹配分析 被引量:3

Correlation between neutrophil subsets and coronary heart disease in patients with SLE: a propensity score matching analysis

摘要 目的倾向性评分匹配法探究系统性红斑狼疮(systemic lupus erythematosus, SLE)患者中性粒细胞亚群与冠心病(coronary heart disease,CHD)病情的相关性。方法选择2018年1月至2019年6月间我院诊治的SLE患者122例,根据是否合并冠心病分为CHD组(33例)和非CHD组(89例),应用倾向性评分匹配法对2组患者进行匹配,比较2组配对患者的临床资料、血液学指标、风湿免疫学指标以及中性粒细胞亚群分布,应用Pearson相关模型分析SLE合并CHD患者的中性粒细胞亚群分布与CHD病情的相关性。结果两组共29对匹配成功,CHD患者的ESR、CRP水平和SLEDAI评分显著高于非CHD组(均P<0.05)。SLE患者的低密度粒细胞(low-density granulocytes,LDGs)水平显著高于健康对照,CHD组患者的LDGs水平明显高于非CHD患者(均P<0.001);健康对照、CHD组和非CHD组患者的正常密度粒细胞(normal-density granulocytes,NDGs)水平间不存在统计学差异(均P>0.05)。SLE合并CHD患者LDGs水平与冠状动脉狭窄程度呈显著正相关(r=0.495,P=0.006);SLE合并CHD患者NDGs水平与冠状动脉狭窄程度无明显相关性(r=-0.168,P=0.332)。结论 SLE合并冠心病患者的LDGs水平显著升高,LDGs水平与CHD病情呈明显正相关。 This study was designed to investigate the correlation between neutrophil subsets and coronary heart disease(CHD) in patients with systemic lupus erythematosus(SLE) using the propensity score matching analysis(PSM).Total of 122 SLE patients diagnosed and treated in our hospital from January 2018 to June 2019 were selected.According to whether the patients had coronary heart disease(CHD), they were divided into CHD group(33 cases) and non-CHD group(89 cases).After match of patients in the two groups using the PSM method,we compared the clinical data, hematological indexes, rheumatological immunological indexes and neutrophil subgroup distribution of the matched patients;Pearson correlation model was used to analyze the correlation between the distribution of neutrophil subpopulations and clinical data in patients with SLE combined CHD.Data showed there were 29 pairs successfully matched in the two groups;the ESR and CRP levels and SLEDAI score in patients with CHD were significantly higher than those in the non-CHD group(P<0.05).The level of low-density granulocytes(LDGs) in patients with SLE were significantly higher than that in healthy controls, and the level of LDGs in patients with CHD were significantly higher than that of non-CHD patients(P<0.001).There was no statistical difference in normal-density granulocytes(NDGs) between healthy controls, CHD, and non-CHD patients(P>0.05).The level of LDGs in patients with SLE complicated with CHD was positively correlated with the degree of coronary artery stenosis(r=0.495, P=0.006);the level of NDGs in patients with SLE combined with CHD had no significant correlation with the degree of coronary artery stenosis(r=-0.168, P=0.332).In conclusion, the level of LDGs in patients with SLE complicated with CHD is significantly increased, and there is a correlation between the level of LDGs and the pathogenetic condition.
