摘要
目的探讨肝素结合蛋白(HBP)、降钙素原(PCT)联合血清淀粉样蛋白A(SAA)对预测严重创伤后呼吸窘迫综合征(acute respiratory distress syndrome,ARDS)发生的诊断价值。方法选取2018年1月至2019年12月因严重创伤在本院重症医学科救治的ARDS患者30例,同时选取严重创伤后非ARDS患者30例作为非ARDS组,测定并统计分析两组患者的血清HBP、PCT、SAA水平,绘制受试者工作特征ROC曲线评价HBP、PCT、SAA以评价对预测严重创伤后ARDS发生的诊断价值。结果非ARDS组、ARDS组患者HBP分别为(7.42±1.51)ng/mL、(11.9±2.86)ng/mL,组间比较差异有统计学意义(P<0.05)。非ARDS组、ARDS组患者PCT分别为(3.41±1.12)ng/mL、(6.32±1.74)ng/mL,组间比较差异有统计学意义(P<0.05),非ARDS组、ARDS组患者SAA分别为(56.94±11.82)mg/L、(89.76±16.57)mg/L,组间比较差异有统计学意义(P<0.05)。结论HBP、PCT、SAA及联合检测为预测严重创伤后ARDS的早期诊断提供参考,具有一定的临床价值。
Objective To investigate the diagnostic value of heparin binding protein(HBP),procalcitonin(PCT)and serum amyloid A(SAA)in the prediction of acute respiratory distress syndrome(ARDS).Methods From January 2018 to December 2019,30 patients with ARDS were treated in the intensive care department of our hospital due to severe trauma,and 30 patients without ARDS after severe trauma were selected as the non ARDS group.The serum levels of HBP,PCT and SAA in the two groups were measured and statistically analyzed,and the receiver operating characteristic(ROC)curve was drawn to evaluate the diagnostic value of HBP,PCT and SAA in predicting the occurrence of ARDS after severe trauma.Results The HBP of non-ards group and ARDS group were(7.42±1.51)ng/mL and(11.9±2.86)ng/mL,respectively,and the difference between the two groups was statistically significant(P<0.05).The PCT values of the non-ARDS group and ARDS group were(3.41±1.12)ng/mL and(6.32±1.74)ng/mL,respectively,and the difference between the two groups was statistically significant(P<0.05).The SAA values of the non-ards group and ARDS group were(56.94±11.82)mg/L and(89.76±16.57)mg/L,respectively,and the difference between the two groups was statistically significant(P<0.05).Conclusion HBP,PCT,SAA and combined detection can provide reference for early diagnosis of ARDS after severe trauma,and have certain clinical value.
