浮针围刺联合微波治疗外阴白斑的临床疗效观察

Observation on the clinical effect of floating needle and circumferential acupuncture combined with microwave in the treatment of vulva leukoplakia

目的探讨浮针围刺联合微波治疗外阴白斑的临床疗效。方法收集2015年4月至2018年9月本院收治的外阴白斑患者120例,随机分为两组,每组60例。浮针组利用毫针对病变局部围刺治疗联合微波照射,每2天1次;氟轻松组应用0.025%氟轻松软膏局部涂抹,每天3次,两组均治疗4周。治疗后,观察不同治疗手段对患者产生的临床疗效,并比较随访3个月后的复发率,对比两组患者治疗前后病变局部组织Ki67表达的变化。结果治疗前,两组患者Ki67表达差异无统计学意义;治疗后,两组患者Ki67表达较治疗前差异均有统计学意义(P<0.05),浮针组与氟轻松组比较差异有统计学意义(P<0.05);随访3个月后,浮针组Ki67表达与氟轻松组比较差异有统计学意义(P<0.05)。浮针组总有效率为91.67%,氟轻松组为63.33%,浮针组明显高于氟轻松组,差异有统计学意义(P<0.05)。随访3个月后,浮针组复发率为25.45%,氟轻松组复发率为39.47%,差异有统计学意义(P<0.05)。结论浮针围刺联合微波治疗外阴白斑,能有效改善局部病变组织的增殖情况,提高临床疗效,增加治愈率,降低复发率,对外阴白斑的治疗具有重要临床价值与意义,值得临床推广与应用。

Objective To investigate the clinical effect of floating acupuncture combined with microwave in the treatment of leukoplakia vulva.Methods 120 patients with vulva leukoplakia from April 2015 to September 2018 in our hospital were collected and randomly divided into two groups,with 60 cases in each group.The floating needling group treated the lesion with local acupuncture combined with microwave irradiation once every two days.In the fluocolone group,0.025%fluocolone ointment was applied topically three times a day.Both groups were treated for 4 weeks.After treatment,the clinical effects of different treatment methods on patients were observed,and the recurrence rate was followed up for 3 months,and the changes of Ki67 expression in the local lesion tissues were compared between the two groups before and after treatment.Results Before treatment,there was no significant difference in Ki67 expression between the two groups.After treatment,there was significant difference between the two groups compared with before treatment(P<0.05).The difference between floatation needle group and fluocolone group was significant(P<0.05).The difference between floatation needle group and fluocolone group was significant(P<0.05).In terms of treatment efficiency,the total effective rate of floatation needle group was 91.67%,and that of fluocolonization needle group was 63.33%.The floatation needle group was significantly higher than that of fluocolonization needle group,and the difference was significant(P<0.05).After 3 months of follow-up,the recurrence rate of the floating needle group was 25.45%,and that of the fluocolone group was 39.47%,with significant difference(P<0.05).Conclusion The combination of floating needle and peri-acupuncture combined with microwave for the treatment of vulva leukoplakia can effectively improve the proliferation of local lesion tissue,improve the clinical efficacy,increase the cure rate and reduce the recurrence rate,which has important clinical value and significance for the treatment of vulva leukoplakia and is worthy of clinical promotion and application.
