腔镜阑尾切除术与传统开放阑尾切除术术后疗效对比分析

Comparative analysis on postoperative curative effect between laparoscopic appendectomy and traditional open appendectomy

目的探究腔镜阑尾切除术(laparoscopic appendectomy,LA)与传统开放阑尾切除术(open appendectomy,OA)术中及术后疗效对比。方法回顾性分析本院2018年7月至2019年6月收治的70例阑尾炎患者的病例资料,根据治疗方法分为A组(n=38)和B组(n=32)。A组采用LA治疗,B组采用OA治疗,比较两组VAS评分、免疫功能指标、术后恢复情况、术后并发症发生率。结果A组术后3 h、1 d VAS评分均低于B组(P<0.05);A组术后24 h白细胞、降钙素原(PCT)、C-反应蛋白(CRP)均低于B组(P<0.05);A组术后首次肛门排气时间、下床时间、住院时间均短于B组(P<0.05);A组患者术后并发症发生率为2.63%,低于B组的18.75%(P<0.05)。结论采用LA治疗阑尾炎,可降低患者VAS评分,改善患者免疫功能指标,缩短患者恢复时间,降低术后并发症发生率,值得临床推广应用。

Objective To investigate and compare postoperative curative effect between laparoscopic appendectomy(LA)and traditional open appendectomy(OA).Methods Retrospective analysis was performed on clinical data of 70 appendicitis patients in the hospital from July 2018 to June 2019.According to different treatment methods,they were divided into group A(n=38)and group B(n=32).Group A was given LA,while group B was given OA.VAS scores,immune function indexes,postoperative recovery,and incidence of postoperative complications were compared between the two groups.Results At 3 h and 1 d after surgery,VAS scores in group A were lower than those in group B(P<0.05).At 24 h after surgery,blood leukocyte,procalcitonin(PCT)and C-reactive protein(CRP)in group A were lower than those in group B(P<0.05).The postoperative first anus exhaust time,leaving bed time and hospitalization time in group A were shorter than those in group B(P<0.05).The incidence of postoperative complications in group A was lower than that in group B(2.63%vs.18.75%)(P<0.05).Conclusion In the treatment of appendicitis,LA can reduce VAS score,improve immune function indexes of patients,shorten their recovery time,and reduce incidence of postoperative complications.It is worthy of clinical application.
