依托咪酯联合丙泊酚麻醉诱导在老年患者手术中的应用效果

Application effect of etomidate combined with propofol anesthesia induction in the operation of elderly patients

目的探讨依托咪酯乳状注射液联合丙泊酚麻醉诱导在老年患者手术中的应用效果。方法选取2018年1月至2019年8月在本院行手术治疗的80例老年患者,按随机数字表法分为两组,各40例。对照组使用丙泊酚行麻醉诱导,实验组使用依托咪酯乳状注射液联合丙泊酚行麻醉诱导。比较两组麻醉指标差异、血流动力学、用药安全性。结果实验组定向力恢复时间、苏醒时间分别为(23.81±0.48)min、(22.49±2.86)min,短于对照组,差异有统计学意义(P<0.05);两组麻醉诱导时间比较差异无统计学意义;与麻醉诱导前(T0)比较,插管前1 min(T1)、插管即刻(T2)、插管后5 min(T3)两组MAP、HR水平均存在不同程度的波动,实验组平均动脉压(MAP)、心率(HR)水平高于对照组,差异有统计学意义(P<0.05);实验组不良反应发生率为7.50%,低于对照组的27.50%,差异有统计学意义(P<0.05)。结论在老年患者手术中使用依托咪酯乳状注射液联合丙泊酚麻醉诱导可有效维持术中血流动力学稳定,缩短术后定向力恢复时间、苏醒时间,降低不良反应发生率,提高手术安全性。

Objective To investigate the effect of etomidate emulsion injection combined with propofol anesthesia induction in elderly patients during surgery.Methods Atotal of 80 elderly patients who underwent surgical treatment in our hospital from January 2018 to August 2019 were selected and they were divided into two groups according to the random number table method,with 40 cases in each group.The control group used propofol for anesthesia induction,and the experimental group used etomidate emulsion injection combined with propofol for anesthesia induction.The difference in anesthesia indexes,hemodynamics,and medication safety were compared between the two groups.Results The orientation recovery time and wake-up time of the experimental group were(23.81±0.48)min and(22.49±2.86)min,which were shorter than those of the control group,the difference was statistically significant(P<0.05).There was no significant difference in the induction time between the two groups.Compared with before induction of anesthesia(T0),there were varying degrees of fluctuations in MAP and HR levels in the two groups at 1 min before intubation(T1),immediately after intubation(T2),and 5 min after intubation(T3).The experimental group MAPand HR levels were higher than those of the control group,and the difference was statistically significant(P<0.05).The adverse reaction rate of the experimental group was 7.50%,which was lower than 27.50%in the control group,and the difference was statistically significant(P<0.05).Conclusion The use of etomidate emulsion injection combined with propofol anesthesia induction in elderly patients can effectively maintain intraoperative hemodynamic stability,shorten postoperative orientation recovery time and wake up time,reduce the incidence of adverse reactions,and improve surgical safety Sex.
