摘要
目的:预防全自动片剂摆药机补充药品环节错误的发生,确保患者用药安全。方法:运用医疗失效模式与效应分析法(HFMEA)评估全自动片剂摆药机补充药品流程中的潜在失效模式及失效后果,进而分析整改措施并实施。没有商品码的药品运用Microsoft BarCode控件生成EAN-13条码,通过药盒的RFID码与药品EAN-13商品码双重匹配核对,用来预防失效模式的再次发生。采用Wilcoxon配对检验比较实施前后失效模式RPN值。结果:应用条码技术,运用医疗失效模式和效应分析法后,影响补充药品环节错误发生的失效模式RPN值较前下降60%(P<0.05)。结论:HFMEA模式的运用以及条码技术的应用,可以有效预防全自动片剂摆药机补药环节错误的发生,提升患者用药安全,在同行中有可推广性。
Objective:To prevent the occurrence of errors in the supplementary medicine in fully-automatic tablet packaging machine to ensure the safety of patients.Methods:The healthcare failure mode and effect analysis(HFMEA)was used to evaluate the potential failure modes and failure consequences in the process of supplementing drug in a fully-automatic tablet packaging machine,and then analyze the rectification measures and implement them.Drugs without a product code used the Microsoft BarCode control to generate an EAN-13 barcode,which were double-checked by the RFID code on the box and the product EAN-13 code to prevent the recurrence of failure mode.The pre-and post-failure mode RPN values were tested by Wilcoxon pairing.Results:After applying the bar code technology and using the medical failure mode and effect analysis method,the failure mode RPN value affecting the errors in the supplementary drug link decreased by 60%,and the difference was statistically significant(P<0.05).Conclusion:The application of HFMEA mode and barcode technology can effectively prevent the occurrence of errors in fully-automatic tablet packaging machine and improve the safety of patients’medication,which can be promoted in the same industry.
