摘要
目的探究C反应蛋白(CRP)及D-二聚体(D-Dimer)在评估急性胰腺炎(AP)严重程度及预后因素的临床意义。方法选择89例急性胰腺炎患者,按照病情严重程度区分为轻度AP组(50例)和重度AP组(39例),分别于起病第1天、第5天和第7天检测2组患者血清CRP及D-Dimer水平,并分别按照是否出现器官衰竭、是否出现胰腺坏死将患者区分为2组,分别比较2组间血清CRP及D-Dimer水平。结果轻度AP组患者第1天、第5天和第7天血清CRP及D-Dimer水平均低于重度AP组(P<0.05);出现器官衰竭的AP患者与未发生器官衰竭的AP患者第1天时CRP及D-Dimer水平对比,差异不具有统计学意义(P>0.05),第5天及第7天出现器官衰竭AP患者上述因子水平明显高于未出现器官衰竭患者(P<0.05);出现胰腺坏死的AP患者第1天、第5天及第7天时CRP及D-Dimer水平均高于未出现胰腺坏死的AP患者(P<0.05)。结论CRP及D-Dimer对AP具有较重要的临床诊断意义,能够区分AP病程严重程度,并对其是否会出现器官衰竭、胰腺坏死等事件具有一定预测价值。
Objective To explore the clinical significance of C-reactive protein(CRP)and D-dimer(D-Dimer)in assessing the severity and prognostic factors of acute pancreatitis(AP).Methods A total of 89 patients with acute pancreatitis were enrolled and divided into mild AP group(50 cases)and severe AP group(39 cases),according to the severity of the disease.The levels of serum CRP and D-Dimer were detected in the two groups on the 1st,5th and 7th day of onset,and the patients were divided into two groups according to whether there was organ failure or pancreatic necrosis,respectively.Serum CRP and D-Dimer levels were compared between groups.Results The serum CRP and D-Dimer levels on the 1st,5th and 7th day of the mild AP group were lower than those in the severe AP group(P<0.05).There was no significant difference in the levels of CRP and D-Dimer between AP patients and AP patients without organ failure on the 1st day(P>0.05).On the 5th and 7th day,the level of these factors was significantly higher in patients with organ failure(P<0.05).The contrast showed that patients with pancreatic necrosis had higher CRP and D-Dimer levels on APs 1st,5th,and 7th than AP patients without pancreatic necrosis(P<0.05).Conclusion CRP and D-Dimer have important clinical diagnostic significance for AP,which can distinguish the severity of AP disease,and whether it will have organ failure,pancreatic necrosis and other events have certain predictive value.
