彩色多普勒超声诊断对肾癌及其血流分型的临床价值分析

Analysis of Clinical Value of Color Doppler Ultrasound Diagnosis on Renal Carcinoma and Blood Flow Type

探讨彩色多普勒超声诊断对肾癌及其血流分型的临床价值。2019年6月—2020年1月通过随访等方法得到病理结果的82例肾癌患者作为研究对象,采用彩色多普勒超声诊断分析患者的肿瘤及血流分型,并将结果与术后病理结果进行比较。同时收集70例肾脏良性占位病变患者的检查结果,使用Logistic逐步回归法筛选影响肾癌与良性肿物诊断的关键变量。结果显示:通过超声诊断报告与患者术后病理结果比较发现,超声诊断正确者78例,正确率为95.12%。透明细胞癌中的血流分型主要为多血流型与血流丰富型,嫌色细胞癌中血流分型主要为少血流型,乳头状细胞癌中的血流分型主要为星点型。Logistic回归分析显示:血管密度及血管面积对肾癌的诊断具有重要价值,回归模型诊断肾癌的灵敏度高达86.3%,准确性高达81.2%。结论:彩色多普勒超声可以较为准确地反映肾癌的病理类型及其血流分型,可为肾癌患者的早期诊断和术前检查提供依据。

The research explores clinical value of color doppler ultrasound diagnosis on renal carcinoma and blood flow type.82 renal carcinoma patients,whose pathology results are gained by follow-up,from June,2019 to January,2020 are regarded as the research objects.Tumor and blood flow of patients are analyzed by color doppler ultrasound,and the results and postoperative pathology results are compared.Examination results of 70 patients with kidney benign space occupying lesion are collected.Key variables of influence factors of renal carcinoma and benign goitre diagnosis are selected through logistic stepwise regression.Through the comparison of ultrasonic diagnosis report and postoperative pathology results,there are 78 patients of correct ultrasound diagnosis,and the correct rate is 95.12%.Blood flow types of clear cell carcinoma are blood flow type and blood rich type.Blood flow types of chromophobe cell tumor are little blood flow.Blood flow types of corpora mammillaria are mainly of point pattern.Logistic regression analysis show that there is important value of blood vessel density and area on the diagnosis of renal tumor.Sensitivity of logistics model diagnosis on renal tumor reaches 86.3%,and the accuracy rate reaches 81.2%.Color doppler ultrasound diagnosis can relatively accurately reflect the pathological type and blood flow type of renal tumor,and it can provide reference for early stage diagnosis and preoperative examination.
