项目管理在缩短骨科住院患者术后尿管留置时间的应用效果观察

Observation of Application Effect of Project Management in Reducing Ureter Postoperation Indwelling Time of Orthopedics Department Inpatients

探讨项目管理在缩短骨科住院患者术后尿管留置时间的应用效果。四川省宜宾市第一人民医院骨科于2020年1月成立缩短骨科住院患者术后尿管留置时间项目管理小组,按照项目管理理论制作查验表,运用便利抽样法,将2020年1—3月在骨科住院手术留置尿管的72例患者作为对照组,将2020年4—6月符合标准的72例患者作为试验组。试验组在观察组留置尿管的基础上,围绕术前评估指导和适应性训练,术后进行拔管指征评估,加强医护交流等,对比项目管理实施前后尿管的留置时间、尿管刺激征发生率、拔管后尿管重置率、患者对尿管管理的满意度。结果显示:试验组术后尿管的留置时间显著短于对照组(P<0.001),尿管刺激征的发生率显著低于观察组(P=0.012),中重度尿管刺激征发生率显著低于对照组(P=0.001)。试验组拔管后尿管的重置率与观察组相比无明显差异(P=0.785),试验组的满意率显著高于对照组(P<0.05)。结论:项目管理方法能有效缩短骨科住院患者术后尿管的留置时间,降低尿管刺激征的发生率,提高患者的满意度。

The research explores application effect of project management in reducing ureter postoperation indwelling time of Orthopedics Department inpatients.Yibin City First People’s Hospital has found project management team of reducing ureter postoperation indwelling time of Orthopedics Department inpatients in Jan.2020.According to project management theory,check list is formulated.And through convenience sampling method,72 patients from Jan.to Mar.2020 are regarded as control group,and 72 patients from Apr.to Jun.2020 are regarded as experimental group.Based on the indwelling catheter in experimental group,in evaluating guidance and acclimatization training before operation,evaluating tube drawing after operation and strengthening communication between doctors and nurses,indwelling time of ureter,ureter stimulating symptoms occurrence rate,ureter resetting rate after tube drawing and the satisfaction of ureter management before and after project management are compared.Indwelling time of ureter after operation in experimental group is significantly shorter than control group(P<0.001).Ureter stimulating symptoms occurrence rate is significantly lower than observation group(P=0.012).Moderate-severe ureter stimulating occurrence rate is significantly lower than control group(P=0.001).There is no statistically significant difference in resetting rate of ureter after tube drawing compared with observation group(P=0.785).The satisfaction rate of experimental group is significantly higher than control group(P<0.05).Project management method can effectively shorten ureter indwelling time of Orthopedics Department inpatients,reduce ureter simulating occurrence rate,and improve patient satisfaction.
