摘要 目的评价应用Ommaya囊进行32P胶体内放射治疗颅咽管瘤的临床效果。方法将Ommaya囊导管植入8例患者的颅咽管瘤囊腔内,其中3例仅行瘤囊摘除术;5例行囊液清除术,待伤口愈合后,将计算所需放射性活度的32P胶体注入瘤囊腔内,为增强治疗效果,可鼓励患者多变换头位,以使瘤囊内的放射性胶体尽可能均匀分布在瘤囊壁的各个部位。定期随访临床疗效,通过CT/MRI等影像检查对比并记录瘤体大小变化。结果经1~4次32P胶体注入瘤囊腔内,内照射治疗3个月至38年的随访观察,8例患者均显示出症状缓解/瘤囊缩小等较显著的疗效。结论应用Ommaya囊进行32P胶体内照射治疗,可有效改善颅咽管瘤患者的生存质量,提高患者生存率。 Objective To evaluate the effect of phosphorus-32 colloid internal radiation therapy via Ommaya cyst catheter installed into cystic craniopharyngioma.Methods The Ommaya cyst catheter was implanted into the cyst of 8 patients with craniopharyngioma.Tumorectomy was performed in 3 cases,while 5 cases only received cystectomy.The required amount of calculated radioactivity of 32P radiocolloid was injected into the cyst cavity of 5 cases after the patient′s wound healed.In order to enhance the treatment effect,patients were encouraged to change their head positions so that the radioactive colloid in the tumor cyst could be distributed evenly on all parts of the tumor cyst wall.The clinical efficacy was followed up regularly to observe and compare the changes in tumor size with CT/MRI and other imaging examinations.Results After 1-4 times of 32P colloid injections into the tumor cyst cavity with a follow-up observation of 3 months to 38 years of internal radiation treatment,8 patients showed significant curative effects,including symptom relief and tumor cyst reduction.Conclusion The application of Ommaya capsule for 32P colloid internal radiation therapy can effectively improve the life quality and increase the survival rate of patients with craniopharyngioma.
