摘要 目的了解1型糖尿病患儿在校期间血糖管理情况,分析其影响因素及在校期间存在的问题,为优化学龄期患儿在校期间的血糖管理提供依据。方法于2018年1月至2018年12月,将温州医科大学附属第二医院、育英儿童医院病程在2~4年,且长期在门诊随访的88名学龄期1型糖尿病患儿作为研究对象,进行问卷调查和实验室检测,以空腹血糖(FPG)和糖化血红蛋白(HbAC1)检测结果评价其血糖控制水平,并分析相关影响因素。结果88名学龄期糖尿病患儿的FPG、HbAC1达标率分别为34.1%和45.5%,其中监护人文化程度、家庭年收入、是否使用胰岛素泵和动态血糖监测与血糖控制密切相关(p<0.05)。结论研究对象在校期间的血糖控制情况不佳,应加强血糖自我管理能力,定期对其进行心理评估与干预,呼吁学校为糖尿病患儿提供健康管理支持,优化血糖控制水平。 Objective To investigate the glycemic management in children with type 1 diabetic mellitus(T1DM)at school,to explore the influencing factors and problems of glycemic management,so as to provide reference for the glycemic management of school-aged T1 DM children at school.Methods From January 2018 to December 2018,88 school-aged T1 DM children with 2-4 years of disease course and regular follow-up in the outpatient of the Second Affiliated Hospital and Yuying Children Hospital of WMU were recruited and given questionnaire survey and laboratory tests.Glycated hemoglobin(HbAC1)and fasting plasma glucose(FPG)were used to evaluate the level of glycemic control,and the influencing factors were analyzed.Results Among the 88 school-aged T1 DM children,the control rate of FPG and HbAC1 were 34.1%and 45.5%respectively.The educational level of guardian,annual household income,insulin pump use,and continuous glucose monitoring use were associated with glycemic control(p<0.05).Conclusion The glycemic contro1 of the participants at school is poor.The ability of self-control should be strengthened,and psychological assessment and intervention should be conducted regularly.It is suggested that school provide health management support for DM students to help with glycemic control.
