复方谷氨酰胺治疗重度急性胰腺炎疗效观察

Observation on the therapeutic effect of compound glutamine on severe acute pancreatitis

目的探讨复方谷氨酰胺治疗重度急性胰腺炎的临床效果。方法将2018年4月—2019年1月在河北省沧州中西医结合医院脾胃肝病科病房治疗的120例重度急性胰腺炎患者随机分为对照组和观察组各60例,对照组患者给予临床常规治疗,观察组则联合给予复方谷氨酰胺治疗,2组均治疗1周。观察2组治疗后临床疗效、T淋巴细胞亚群、肠道气体容量积分(GVS积分)、APACHEⅡ评分及血清炎性因子C反应蛋白(CRP)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平变化。结果观察组治疗后总有效率明显高于对照组(P<0.05);治疗后2组CD3+、CD4+、CD4+/CD8+比值均较治疗前明显升高(P均<0.05),CD8+则较治疗前明显下降(P均<0.05),观察组各指标变化幅度明显高于对照组(P均<0.05);治疗后2组GVS积分、APACHEⅡ评分及血清CRP、IL-6、TNF-α水平均较治疗前明显下降(P均<0.05),观察组各指标均明显低于对照组(P均<0.05)。结论复方谷氨酰胺用于治疗重度急性胰腺炎患者能有效抑制机体炎症反应,改善免疫功能,疗效确切。

Objective It is to explore the clinical effect of compound glutamine in the treatment of severe acute pancreatitis.Methods 120 patients with severe acute pancreatitis treated in Cangzhou Hospital of Integrated Traditional Chinese and Western Medicine from April 2019 to January 2019 were randomly divided into a control group and an observation group with 60 cases in each group.The control group received clinical routine treatment,and the observation group was additionally treated with traditional Chinese medicine.Both groups were treated for 1 week.The clinical efficacy was evaluated,and the changes of T lymphocyte subsets,intestinal gas volume score(GVS)score,APACHEⅡscore,the levels of serum inflammatory factor C-reactive protein(CRP),interleukin(IL-6),and tumor necrosis factor-α(TNF-α)after treatment were observed in the two groups.Results The total effective rate of the observation group after treatment was significantly higher than that of the control group(P<0.05).After treatment,the levels of CD3+,CD4+,CD4+/CD8+in the two groups were significantly higher than those before treatment(P<0.05),while CD8+was significantly lower than that before treatment(P<0.05),the change of each index of the observation group was significantly higher than that of the control group(P<0.05).After treatment,the GVS score,APACHEⅡscore and the levels of serum CRP,IL-6,and TNF-αin the two groups were significantly decreased compared with before treatment(P<0.05),the indicators of the observation group were significantly lower than those of the control group(P<0.05).Conclusion The compound glutamine used can effectively inhibit the body's inflammatory response,improve the immune function,and has a definite effect in the treatment of patients with severe acute pancreatitis.
