固本扶正醒脑方对急性脑出血患者血肿吸收、神经功能及细胞炎性因子的影响

Effect of Guben Fuzheng Xingnao Decoction on hematoma absorption,neurological function and cytoinflammatory factor in patients with acute cerebral hemorrhage

目的探讨固本扶正醒脑方对急性脑出血患者血肿吸收、神经功能及细胞炎性因子的影响。方法选择2017年1月—2019年1月天津中医药大学第一附属医院收治的80例急性脑出血患者,随机分为观察组和对照组,每组40例。对照组给予西医常规对症治疗,观察组在对照组治疗基础上给予固本扶正醒脑方治疗,观察比较2组治疗前后单项症状积分(头晕、口眼歪斜、语言表达不利、肢体不遂、乏力)、血肿吸收状况(血肿体积、水肿带体积)、意识状态(GCS评分)、神经功能(NIHSS评分)、炎症因子[白细胞介素-10(IL-10)、白细胞介素-18(IL-18)、高敏C反应蛋白(hs-CRP)、肿瘤坏死因子-α(TNF-α)]水平的变化,比较2组临床疗效。结果2组治疗后头晕、口眼歪斜、语言表达不利、肢体不遂、乏力积分及血肿体积、水肿带体积、NHISS评分均较治疗前明显降低(P<0.05),且观察组均明显低于对照组(P<0.05);2组治疗后GCS评分及血清IL-10水平均较治疗前明显升高(P<0.05),且观察组明显高于对照组(P<0.05);2组治疗后血清IL-18、hs-CRP、TNF-α水平均较治疗前明显降低(P<0.05),且观察组明显低于对照组(P<0.05)。观察组总有效率为92.5%(37/40),明显高于对照组的75.0%(30/40)(P<0.05)。结论固本扶正醒脑方治疗急性脑出血疗效确切,能够有效抑制机体炎症反应,促进血肿吸收,改善患者神经功能,有助于临床预后转归。

Objective It is to investigate the effects of Guben Fuzheng Xingnao Decoction on hematoma absorption,neurological function and cytoinflammatory factor in patients with acute cerebral hemorrhage.Methods 80 patients with acute cerebral hemorrhage admitted to the First Affiliated Hospital of Tianjin University of Tradition Chinese Medicine from January 2017 to January 2019 were randomly divided into observation group and control group,with 40 cases in each group.The control group was treated with routine symptomatic treatment of western medicine,the observation group was treated with Guben Fuzheng Xingnao Decoction on the basis of treatment in the control group.The changes of single symptom scores(dizziness,distorted mouth and eyes,unfavorable language expression,limb hemiplegia,fatigue),absorption of hematoma(volume of hematoma,volume of edema zone),state of consciousness(GCS scores),neurological function(NIHSS scores),and inflammatory factors(IL-10,IL-18,hs-CRP,TNF-α)before and after treatment were observed,and the clinical curative effect was compared in the two groups.Results After treatment,the scores of dizziness,distorted mouth and eyes,unfavorable language expression,limb hemiplegia,fatigue,and volume of hematoma,volume of edema zone,NIHSS scores were significantly reduced compared with before treatment(P<0.05),and these indexes of the observation group were lower than those of the control group(P<0.05).After treatment,the levels of IL-10 and the GCS scores in both groups were significantly increased compared with before treatment(P<0.05),while the levels of IL-18,hs-CRP and TNF-αwere significantly decreased(P<0.05),the improvement of the above inflammatory factors in the observation group was better than that in the control group(P<0.05).The total effective rate of the treatment group was 92.5%(37/40),which was significantly higher than 75.0%(30/40)of the control group(P<0.05).Conclusion Guben Fuzheng Xingnao Decoction is effective in treating acute cerebral hemorrhage,it can effectively inhibit local inflammation,promote hematoma absorption,improve neurological function and contribute to clinical prognosis.
