摘要
目的:研究多普勒检测脐动脉(UA)、大脑中动脉(MCA)、静脉导管(DV)血流参数变化运用于宫内发育迟缓(IUGR)胎儿不良妊娠结局评估中的价值。方法:选择我院87例IUGR孕妇作为研究对象,分别采取多普勒进行UA、MCA、DV血流参数检测,同时观察所有研究对象的妊娠结局。结果:血流正常组的UA血流参数S/D、PI及RI均低于血流异常组,差异有统计学意义(P<0.05)。血流正常组的MCA血流参数S/D、PI及RI均高于血流异常组,差异有统计学意义(P<0.05)。血流正常组的DV血流参数S/D、PI及RI均低于血流异常组,差异有统计学意义(P<0.05)。UA、MCA、DV血流正常组在剖宫产、围生期死亡及Apart评分≤7分方面与血流异常组相比差异有统计学意义(P<0.05)。结论:多普勒超声下检测UA、MCA、DV血流参数变化能够有效预评估IUGR胎儿的妊娠结局,尽早给予相关干预。
Objective To study the value of Doppler flow parameters of umbilical artery(UA),middle cerebral artery(MCA)and venous catheter(DV)in evaluating fetal adverse pregnancy outcomes in intrauterine growth retardation(IUGR).Method Eighty-seven IUGR pregnant women admitted to our hospital were selected as the study subjects.The blood flow parameters of UA,MCA and DV were measured by Doppler,and the pregnancy outcomes of all the subjects were observed.Results UA blood flow parameters S/D,PI and RI in normal blood flow group were lower than those in abnormal blood flow group,the difference was statistically significant(P<0.05).The blood flow parameters S/D,PI and RI of MCA in normal blood flow group were higher than those in abnormal blood flow group,the difference was statistically significant(P<0.05).DV blood flow parameters S/D,PI and RI in normal blood flow group were lower than those in abnormal blood flow group,the difference was statistically significant(P<0.05).UA,MCA,DV blood flow group normal group in cesarean section,perinatal mortality and Apart score≤7 points compared with abnormal blood flow group were significantly different(P<0.05).Conclusion Detection of UA,MCA and DV blood flow parameters under Doppler ultrasound can effectively predict the pregnancy outcome of IUGR fetus and give relevant intervention as soon as possible.
