糖尿病合并重症肺炎患者行抗生素降阶梯治疗的疗效、安全性研究

Study on the Efficacy and Safety of Antibiotic De-escalation Therapy in Patients with Diabetes and Severe Pneumonia

目的研究抗生素降阶梯治疗方案对糖尿病合并重症肺炎的安全性和有效性。方法选取2019年1月—2020年1月期间于该院医治糖尿病合并重症肺炎的60例患者,以治疗方法的不同为分组依据,对照组、研究组各30例。对照组常规使用抗生素,研究组降阶梯使用抗生素。组间对比血糖指标、肺功能指标、不良反应发生率。结果治疗后研究组的空腹血糖、糖化血红蛋白、餐后2 h血糖均比对照组低(t=7.623、3.372、6.014,P<0.05),用力肺活量、第1秒用力呼气容积及二者比值均比对照组高(t=5.718、5.020、4.853,P<0.05),不良反应发生率为3.33%,比对照组的20.00%低,差异有统计学意义(χ2=4.043,P<0.05)。结论抗生素降阶梯治疗方案对糖尿病合并重症肺炎患者具有明显的疗效和安全性。

Objective To study the safety and effectiveness of antibiotic de-escalation therapy for diabetes with severe pneumonia.Methods A total of 60 patients with diabetes and severe pneumonia who were treated in the hospital from January 2019 to January 2020 were selected.Based on the different treatment methods,30 patients were divided into the control group and the study group.The control group routinely used antibiotics,and the study group used antibiotics down the ladder.The blood glucose indexes,lung function indexes,and the incidence of adverse reactions were compared between the groups.Results After treatment,the fasting blood glucose,glycosylated hemoglobin,and blood glucose 2 h postprandial of the study group were all lower than those of the control group(t=7.623,3.372,6.014,P<0.05),forced vital capacity,forced breathing volume in the first second,and the ratio of the two were higher than that of the control group(t=5.718,5.020,4.853,P<0.05),the incidence of adverse reactions was 3.33%,which was lower than 20.00%in the control group,and the difference was statistically significant(χ2=4.043,P<0.05).Conclusion The antibiotic de-escalation treatment program has obvious efficacy and safety in patients with diabetes and severe pneumonia.
