化痰散结汤对晚期胃癌化疗增效、毒副反应及免疫功能的影响

Effect of Huatan Sanjie Decoction on Chemotherapy Synergism,Toxic and Side Effects and Immune Function for Advanced Gastric Cancer

目的:观察化痰散结汤对晚期胃癌化疗增效、毒副反应及免疫功能的影响。方法:纳入我院2016年2月~2018年12月期间92例晚期胃癌患者,随机分为对照组、观察组各46例,两组均实施XELOX化疗方案,观察组在此基础上加服化痰散结汤。治疗4个疗程后,比较两组患者治疗总缓解率,治疗期间毒副反应发生情况,治疗前后免疫功能指标[T细胞亚群(CD3、CD4、CD4/CD8)、NK细胞(CD16、CD57)],炎性因子[白细胞介素-6(IL-6)、白细胞介素-8(IL-8)、C反应蛋白(CRP)]变化。结果:治疗4个疗程后,两组患者总缓解率差异无统计学意义(P>0.05),观察组患者总稳定率高于对照组(P<0.05);治疗期间,观察组患者肝肾功能异常、白细胞减少、神经毒性、胃肠道反应总发生率低于对照组(P<0.05);治疗4个疗程后,对照组患者CD3、CD4、CD4/CD8、CD16、CD57均较治疗前降低(P<0.05),观察组较治疗前无显著变化(P>0.05),两组差异均有统计学意义(P<0.05);两组患者IL-6、IL-8、CRP水平均较治疗前降低,且观察组低于对照组(P<0.05)。结论:化痰散结汤应用于晚期胃癌化疗患者疗效确切,可改善患者免疫功能,缓解机体炎症反应,并一定程度上减轻化疗的毒副反应。

Objective:To observe the effect of Huatan Sanjie Decoction on chemotherapy synergism,toxic and side effects and immune function of advanced gastric cancer.Methods:A total of 92 patients with advanced gastric cancer from February 2016 to December 2018 in our hospital were included and randomly divided into controlled group and observation group,with 46 cases in each group.The two groups were treated with XELOX chemotherapy regimen while the observation group was treated with Huatan Sanjie Decoction on this basis.After 4 courses of treatment,the total remission rate of treatment,occurrence of toxic and side effects during treatment and immune function indicators[T cell subsets(CD3,CD4,CD4/CD8),NK cells(CD16,CD57)]and inflammatory factors[interleukin-6(IL-6),interleukin-8(IL-8),C-reactive protein(CRP)]before and after treatment were compared between the two groups.Results:After 4 courses of treatment,there was no significant difference in the total remission rate between the two groups(P>0.05),and the total stability rate in the observation group was higher than that in the controlled group(P<0.05).During treatment,the total incidence rate of abnormal liver and kidney function,leukopenia,neurotoxicity and gastrointestinal reactions in the observation group were lower than those in the controlled group(P<0.05).After 4 courses of treatment,the CD3,CD4,CD4/CD8,CD16 and CD57 in the controlled group were all decreased compared with those before treatment(P<0.05),and there were no significant changes in the observation group compared with those before treatment(P>0.05),and the differences between the two groups were statistically significant(P<0.05).The levels of IL-6,IL-8 and CRP in the two groups were decreased compared with those before treatment,and the levels in the observation group were lower than those in the controlled group(P<0.05).Conclusion:Huatan Sanjie Decoction has exact efficacy on patients with advanced gastric cancer chemotherapy.It can improve the immune function,alleviate the inflammatory response of body,and relieve the toxic and side effects of chemotherapy to a certain extent.
