宣肺通窍汤加味治疗分泌性中耳炎风寒外袭证的疗效及对血清炎症因子水平的影响

Effect of Using Modified Xuanfei Tongqiao Decoction in the Treatment of Secretory Otitis Media with Wind-cold Exterior Attack Syndrome and Its Influence on the Level of Serum Inflammatory Factors

目的:观察宣肺通窍汤加味联合西药治疗分泌性中耳炎(SOM)风寒外袭证的疗效及对患者血清炎症因子水平的影响。方法:选取2017年10月~2019年10月本院收治的100例SOM患者为对象,采用随机数字法分为对照组(n=50)和观察组(n=50),对照组给予醋酸泼尼松片治疗,观察组在对照组基础上给予宣肺通窍汤加味治疗,比较两组临床疗效、中医症候积分、鼓室导抗图、血清炎症因子[白细胞介素-8(IL-8)、血小板活化因子(PAF)、肿瘤坏死因子-α(TNF-α)]水平及不良反应发生率。结果:观察组总有效率高于对照组(92.00%VS 70.00%)(P<0.05);治疗后观察组中医症候积分低于对照组(P<0.05);观察组鼓室导抗图优于对照组(P<0.05);观察组血清IL-8、PAF、TNF-α水平均低于对照组(P<0.05);两组不良反应发生率比较差异无统计学意义(P>0.05)。结论:宣肺通窍汤加味联合西药治疗SOM疗效显著,可改善临床症状和听力功能,降低血清炎症因子水平,且安全性较高。

Objective:To explore the effect of Prednisone Acetate Tablets combined with Modified Xuanfei Tongqiao Decoction for treating secretory otitis media(SOM)with wind-cold exterior attack syndrome and its influence on the level of serum inflammatory factors.Methods:100 patients with SOM admitted to the hospital between October 2017 and October 2019 were selected as subjects.They were divided into controlled group(n=50)and observation group(n=50)by random number method.Both groups were treated with Prednisone Acetate Tablets,and the observation group were additionally treated with Modified Xuanfei Tongqiao Decoction.The clinical effect,TCM symptom scores,tympanogram,levels of serum inflammatory factors[interleukin-8(IL-8),platelet activating factor(PAF),tumor necrosis factor-α(TNF-α)]and incidence of adverse reactions were compared between the two groups.Results:The total effective rate of the observation group was higher than that of the controlled group(92.00%VS 70.00%)(P<0.05).TCM symptom scores of the observation group were lower than those of the controlled group after treatment(P<0.05).Levels of serum IL-8,PAF and TNF-αin the observation group were lower than those in the controlled group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Prednisone Acetate Tablets combined with Modified Xuanfei Tongqiao Decoction is markedly effective in the treatment of SOM.It can improve clinical symptoms and hearing function and reduce levels of serum inflammatory factors,with high safety.
