摘要
目的分析内皮素-1(ET-1)、基质金属蛋白酶-9(MMP-9)表达水平与声带息肉样病变的关系。方法选取2016年3月至2019年3月我院收治的声带息肉样病变患者80例(观察组)及同期声带正常患者40例(对照组)为研究对象,采用免疫组织化学染色法测定两组声带组织标本中ET-1、MMP-9表达水平,对比两组声学参数[基频(F0)、基频微扰(Jitter)、振幅微扰(Shimmer)、最大发声时间(MPT)],分析声带息肉组织ET-1、MMP-9表达水平与其声学参数的关系。结果观察组声带组织标本中ET-1、MMP-9表达水平高于对照组(P<0.05);观察组F0、MPT低于对照组,而Jitter、Shimmer高于对照组(P<0.05);声带息肉样病变患者中,ET-1阳性表达者F0、MPT低于ET-1阴性表达者,而Jitter高于ET-1阴性表达者,MMP-9阳性表达者MPT低于MMP-9阴性表达者,而Jitter、Shimmer高于MMP-9阴性表达者(P<0.05);相关分析显示,声带息肉样病变患者声带组织中ET-1表达水平与F0、MPT呈负相关,与Jitter呈正相关,MMP-9表达水平与MPT呈负相关,与Jitter、Shimmer呈正相关,ET-1也与MMP-9表达水平呈正相关(P<0.05)。结论声带息肉样病变患者声带组织中ET-1、MMP-9呈高表达,且两者共同作用影响声学参数,同时抑制ET-1、MMP-9的表达有望作为一种更好的声带息肉样病变治疗方案。
Objective To analyze the relationship between the expression levels of endothelin-1(ET-1)and matrix metalloproteinase-9(MMP-9)and polypoid lesions of the vocal cord.Methods A total of 80patients with polypoid lesions of the vocal cord(observation group)who were admitted and treated in the hospital during the period from March 2016to March 2019and 40patients with normal vocal cords(control group)were selected as the research subjects.The expression levels of ET-1and MMP-9in vocal cord tissue specimens of both groups were determined by immunohistochemical staining.The acoustic parameters[fundamental frequency(F0),Jitter,Shimmer,maximum phonatory time(MPT)]were compared between the two groups.The relationship between the expression levels of ET-1and MMP-9 in vocal cord polyps and the acoustic parameters was analyzed.Results The expression levels of ET-1and MMP-9in vocal cord tissue specimens of observation group were higher than those in control group(P<0.05).F0and MPT in the observation group were lower than those of the control group,while Jitter and Shimmer in the observation group were higher than those of the control group(P<0.05).Among patients with polypoid lesions of the vocal cord,those with ET-1positive expression had lower F0and MPT but higher Jitter than those with ET-1negative expression.Those with MMP-9positive expression had lower MPT but higher Jitter and Shimmer than those with MMP-9negative expression(P<0.05).Correlation analysis showed that the expression level of ET-1in vocal cord tissues of patients with polypoid lesions of the vocal cord was negatively correlated with F0and MPT,but was positively correlated with Jitter.The expression level of MMP-9was negatively correlated with MPT but was positively correlated with Jitter and Shimmer.ET-1was positively correlated with the expression level of MMP-9(P<0.05).Conclusion ET-1and MMP-9are highly expressed in vocal cord tissues of patients with polypoid lesions of the vocal cord,both of which affect the acoustic parameters.Additionally,inhibiting the expression of ET-1and MMP-9is expected to be a better treatment plan for polypoid lesions of the vocal cord.
