常州某三级医院糖尿病患者血流感染临床特点的回顾性分析

摘要 目的分析糖尿病合并血流感染者的临床特征及病原菌特点,以指导临床治疗。方法回顾性分析2016年1月~2018年12月于常州市武进人民医院诊治的总计295例确诊为血流感染患者的临床资料。结果糖尿病合并血流感染患者中女性比例高于无糖尿病患者(P<0.05);糖尿病患者总住院时间长于无糖尿病患者(P<0.05);糖尿病血流感染患者转归好于无糖尿病患者(P<0.05);糖尿病血流感染患者大肠埃希氏菌比例高于无糖尿病患者(P<0.05);糖尿病患者肺部感染来源比例低于无糖尿病患者,尿路感染比例高于无糖尿病患者(P<0.05);糖尿病患者D-二聚体水平低于无糖尿病患者,血小板计数、糖化血红蛋白水平高于无糖尿病患者,差异均有统计学意义(P<0.05)。结论糖尿病患者更易发生血流感染;女性糖尿病患者更易发生血流感染,尤其是继发于尿路感染的大肠埃希菌血流感染。 Objective To analyze the clinical features and pathogenic bacteria features of diabetes mellitus complicated with bloodstream infection,so as to guide the clinical treatment.Methods From January 2016 to December 2018,the clinical data of 295 patients diagnosed with bloodstream infection admitted to Changzhou Wujin People's Hospital were analyzed retrospectively.Results The proportion of female in diabetic patients complicated with bloodstream infection was higher than that in non-diabetic patients(P<0.05).The total hospitalization time of diabetic patients was longer than that of non-diabetic patients(P<0.05).The outcome of diabetic patients with bloodstream infection was better than that in non-diabetic patients(P<0.05).The proportion of Escherichia coli of diabetic patients was lower than that in non-diabetic patients(P<0.05).The incidence of pulmonary infection in diabetic patients was lower than that in non-diabetic patients,and the incidence of urinary tract infection was higher than that in non-diabetic patients(P<0.05).The level of D-dimer in diabetic patients was lower than that in nondiabetic patients,and the platelet count and glycosylated hemoglobin level were higher than those in non-diabetic patients,the differences were statistically significant(P<0.05).Conclusion Diabetic patients are more prone to bloodstream infection,and the prognosis is worse than that of patients without risk factors.Female diabetic patients are more prone to bloodstream infection,especially Escherichia coli bloodstream infection secondary to urinary tract infection.
