卡前列素氨丁三醇联合子宫背带式缝合术治疗宫缩乏力性产后出血对照观察

Controlled observation of carboprost tromethamine combined with uterine brace suture in the treatment of postpartum hemorrhage caused by uterine contraction

目的探讨卡前列素氨丁三醇联合子宫背带式缝合术在宫缩乏力性产后出血患者中的应用效果.方法将92例宫缩乏力性产后出血患者按治疗方案分为两组,各46例.对照组给予卡前列素氨丁三醇联合子宫动脉结扎术治疗,观察组给予卡前列素氨丁三醇联合子宫背带式缝合术治疗,观察住院全程.比较两组疗效、手术指标(手术时间、住院时间及术后24 h血红蛋白下降值)、出血(术中、术后2 h、术后24 h)、并发症及生活质量综合评定问卷评分(分娩前、出院前).结果观察组总有效率显著高于对照组(P<0.05),并发症发生率显著低于对照组(P<0.01).观察组手术时间、住院时间显著短于对照组(P<0.01),术后24 h血红蛋白下降值显著小于对照组(P<0.01),术中、术后2 h、术后24 h出血量显著少于对照组(P<0.01).出院前观察组生活质量综合评定问卷评分显著高于对照组(P<0.01).结论卡前列素氨丁三醇联合子宫背带式缝合术治疗宫缩乏力性产后出血效果显著,可明显缩短手术时间,减少出血,降低并发症发生率,提高患者术后生活质量.

Objective To investigate the effect of carboprost tromethamine combined with uterine brace.suture in the treatment of postpartum hemorrhage caused by uterine contraction.Methods A total of 92 patients with postpartum hemorrhage due to uterine contractions were divided into two groups according to different treatment options,46 cases each.The control group was treated with car boprost tromethamine and uterine artery ligation.The observation group was treated with carboprost tromethamine and uterine brace suture.Observe the whole process of hospitalization.The efficacy,surgical indicators(operating time.hospital stay,and 24-h hemoglobin decline value after surgery),blood loss(intraoperative,2 h after surgery,24 h after surgery).complications and quality of life comprehensive assessment questionnaire scores were compared between the two groups(before delivery,before discharge).Results The total ef-fective rate of the observation group was significantly higher than that of the control group(P<0.05),and the incidence of complications was significantly lower than that of the control group(P<0.01).The operation time and hospital stay of the observation group were significantly shorter than those of the con-trol group(P<0.01),the hemoglobin decline value at 24 h after operation was significantly lower than that in the control group(P<0.01),and the blood loss during the operation,2 h after the operation,and 24 h after the operation was significant less than the control group(P<0.01).The GQOLI-74 score of the observation group before discharge was signifi-cantly higher than that of the control group(P<0.01).Conclusions Carboprost tromethamine combined with uterine brace suture has a significant effect on the treatment of postpartum hemorrhage caused by uterine attenuation.It can significantly short-en the operation time,reduce the amount of bleeding,reduce the incidence of complications,and improve the quality of life of patients after surgery.
