老年糖尿病肾病患者的血清维生素D水平及其影响因素分析

Analysis of Serum Vitamin D Level and Its Influencing Factors in Elderly Diabetic Nephropathy

目的分析老年糖尿病肾病患者的血清维生素D水平及其影响因素。方法随机选择2018年1月—2020年1月于该院接收治疗的90例老年2型糖尿病患者作为研究对象,均行尿白蛋白肌酐比值检测,并根据检测结果将90例患者分为糖尿病肾病组(41例)和单纯糖尿病组(49例),比较两组患者的胱抑素C(CYSC)、糖化血红蛋白(HbA1c)、25-羟基维生素D3[25-(OH)VD3]水平,并分析影响组间25-(OH)VD3水平存在差异的因素。结果单纯糖尿病组的CYSC(1.27±0.42)mg/L、HbA1c(7.12±0.63)%均较糖尿病肾病组(2.67±0.31)mg/L、(8.87±1.21)%低(t=2.330、2.110,P<0.05),其25-(OH)VD3水平(57.34±9.71)mmol/L较糖尿病肾病组(36.44±9.56)mmol/L高(t=9.670,P<0.05);糖尿病肾病患者的25-(OH)VD3水平与其病程、年龄、体重指数、空腹血糖(FBG)、甘油三酯(TG)有关,并呈负相关性(r=-0.485、-0.551、-0.304、-0.312、-0.368,P<0.05)。结论老年糖尿病肾病患者的血清维生素D水平呈现降低趋势,而病程、年龄、体重指数、FBG、TG是导致其下降的主要因素。

Objective To analyze the serum vitamin D level and its influencing factors in elderly patients with diabetic nephropathy.Methods Ninety patients with type 2 diabetes who were treated in the hospital from January 2018 to January 2020 were random selected as the research objects.All patients were tested for the ratio of urine albumin to creatinine,and 90 patients were divided into diabetic nephropathy group based on the test results(41 cases)and simple diabetes group(49 cases),compare the levels of cystatin C(CYSC),glycosylated hemoglobin(HbA1c),25-hydroxyvitamin D3[25-(OH)VD3]between the two groups,and analyze the effects Factors for differences in 25-(OH)VD3 levels between groups.Results The CYSC(1.27±0.42)mg/L and HbA1c(7.12±0.63)%of the simple diabetes group were lower than those of the diabetic nephropathy group(2.67±0.31)mg/L,(8.87±1.21)%(t=2.330,2.110,P<0.05),the 25-(OH)VD3 level(57.34±9.71)mmol/L was higher than that of the diabetic nephropathy group(36.44±9.56)mmol/L(t=9.670,P<0.05);the 25-(OH)VD3 level of patients with diabetic nephropathy were related to the course of disease,age,body mass index,fasting blood glucose(FBG),triglycerides(TG),and are negatively correlated(r=-0.485,-0.551,-0.304,-0.312,-0.368,P<0.05).Conclusion The serum vitamin D level of elderly patients with diabetic nephropathy shows a decreasing trend,and the course of disease,age,body mass index,FBG,and TG are the main factors leading to the decrease.
