B型钠尿肽在小儿重症肺炎诊疗及预后评价中的应用

Application of B-type Natriuretic Peptide in Diagnosis,Treatment and Prognosis Evaluation of Severe Pneumonia in Children

目的探讨B型钠尿肽(BNP)检测结合对于小儿重症肺炎诊疗及预后评价的参考价值。方法方便选取2018年4月—2019年9月期间于该院接受诊疗的58例小儿重症肺炎患儿、60例轻症肺炎患儿和60名健康儿童作为研究对象,进行分组对照研究,在临床诊疗中,进行BNP、超敏C反应蛋白(hs-CRP)检测,观察其检测结果,结合患者的预后进行评价。结果重症肺炎患儿的BNP、hs-CRP分别为(309.44±15.81)pg/mL和(3.08±0.62)mg/L,显著高于轻症肺炎患儿和健康儿童(P<0.05),而轻症肺炎患儿的BNP、hs-CRP水平高于健康儿童(P<0.05)。与治疗前相比,重症肺炎患儿的BNP水平[(147.23±5.96)pg/mL<(309.44±15.81)pg/mL(t=73.742)]、hs-CRP水平[(2.02±0.37)mg/L<(3.08±0.62)pg/mL(t=11.277)]以及急性生理学及慢性健康状况评分系统(APACHEⅡ)评分[(16.93±1.27)分<(20.94±1.65)分(t=14.793)]相对更低(P<0.05)。结论在小儿重症肺炎诊断中,BNP的检测结果具有重要的参考价值,能够准确反映出病情的发生和进展情况。而在疾病的临床治疗中,BNP检测结果可以作为预后评价的重要依据。

Objective To explore the reference value of the combination of B-type natriuretic peptide(BNP)detection in the diagnosis,treatment and prognosis evaluation of severe pneumonia in children.Methods 58 children with severe pneumonia,60 children with mild pneumonia,and 60 healthy children who were diagnosed and treated in the hospital from April 2018 to September 2019 were convenient selected as the research objects to conduct a group-controlled study.During diagnosis and treatment,BNP and high-sensitivity C-reactive protein(hs-CRP)were tested,the test results were observed,and the patient’s prognosis was evaluated.Results The BNP and hs-CRP of children with severe pneumonia were(309.44±15.81)pg/mL and(3.08±0.62)mg/L,respectively,which were significantly higher than those of children with mild pneumonia and healthy children(P<0.05),while those of mild pneumonia The levels of BNP and hs-CRP in children with pneumonia were higher than those in healthy children(P<0.05).Compared with before treatment,the BNP level of children with severe pneumonia[(147.23±5.96)pg/mL<(309.44±15.81)pg/mL(t=73.742)],hs-CRP level[(2.02±0.37)mg/L<(3.08±0.62)pg/mL(t=11.277)]and acute physiology and chronic health status scoring system(APACHEⅡ)score[(16.93±1.27)points<(20.94±1.65)points(t=14.793)]Relatively lower(P<0.05).Conclusion In the diagnosis of severe pneumonia in children,the detection results of BNP have important reference value and can accurately reflect the occurrence and progress of the disease.In the clinical treatment of diseases,BNP test results can be used as an important basis for prognostic evaluation.
