多西他赛联合奥沙利铂及替吉奥对晚期胃癌血清肿瘤标志物水平及预后的影响

Effect of TXT combined with L-OHP and tegafur on serum tumor markers and prognosis in patients with advanced gastric cancer

目的探讨多西他赛(TXT)联合奥沙利铂(L-OHP)及替吉奥(S-1)在晚期胃癌治疗中的疗效。方法选取2016年1月至2018年1月阳江市阳东区人民医院晚期胃癌病人89例,根据最终选取的治疗方案分为观察组(n=41)和对照组(n=48),观察组给予TXT联合L-OHP及S-1治疗,对照组给予TXT、顺铂、5-氟尿嘧啶(DCF)方案一线治疗,观察两组近期疗效、不良反应、卡氏功能评分(Karnofaky)及预后,检测治疗前后神经元特异性烯醇化酶(NSE)、糖类抗原125(CA125)和糖类抗原199(CA199)水平。结果观察组和对照组近期疗效、不良反应比较差异无统计学意义(P>0.05);观察组治疗后NSE、CA125和CA199分别为(24.38±7.11)mg/L、(32.28±12.81)U/mL和(42.20±11.28)U/mL,明显低于对照组(35.57±8.10)mg/L、(45.50±15.44)U/mL、(50.15±13.03)U/mL(P<0.05);观察组治疗后Karnofaky评分为(78.82±3.80)分,明显高于对照组(73.30±3.11)分(P<0.05);观察组和对照组中位生存时间分别为11个月和10个月,差异无统计学意义(P>0.05)。结论TXT联合L-OHP及S-1治疗晚期胃癌有较好的效果,能明显降低病人NSE、CA125和CA199水平,改善病人生存质量。

Objective To investigate the efficacy of docetaxel(TXT),oxaliplatin(L-OHP)and tegafur in the treatment of advanced gastric cancer.Methods A total of 89 patients with advanced gastric cancer admitted to People’s Hospital of Yangdong District in Yangjiang City from January 2016 to January 2018 were selected,and divided into the observation group(n=41)and the control group(n=48)according to the final treatment plan.The observation group was treated with TXT combined with L-OHP and tegafur,while the control group was treated with DCF regimen.The short-term efficacy,adverse reactions,Karnofaky score and prognosis of the two groups were observed,the levels of neuron specific enolase(NSE),carbohydrate antigen 125(CA125)and carbohydrate antigen antigen 199(CA199)before and after treatment were detected.Results There were no significant difference in short-term efficacy and adverse reactions between the observation group and the control group(P>0.05);After treatment,NSE,CA125 and CA199 in the observation group were(24.38±7.11)mg/L,(32.28±12.81)U/mL and(42.20±11.28)U/mL,which were significantly lower than those in the control group(P<0.05);The Karnofaky score in the observation group after treatment was(78.82±3.80),which was significantly higher than that in the control group(P<0.05);The median survival time of the observation group and the control group were 11 months and 10 months,andthere difference was no statistical significance(P>0.05).Conclusion TXT combined with L-OHP and tegafur has a good effect in the treatment of advanced gastric cancer,which can significantly reduce the levels of NSE,CA125 and CA199,and improve the quality of life of patients.
