运用距下关节制动器Hyprocure治疗儿童柔韧性平足症的临床疗效

摘要 目的:探讨运用距下关节制动器Hyprocure治疗儿童柔韧性平足症的临床疗效。方法:2016年1月-2018年2月收治儿童柔韧性平足症患者12例(18足),采用放置距下关节制动器Hyprocure实施距下关节制动术,比较术前、术后距骨第一跖骨角(Meary角)、跟骨倾斜角(Pitch角)、距舟覆盖角(TCA)变化,采用AOFAS踝与后足功能评分进行疗效评价。结果:12例均获得随访,切口Ⅰ期愈合,患者在穿鞋、步行时,患足疼痛较术前明显减轻,负重正位X线片上距骨第一跖骨角(T1MT),负重侧位X线片上跟骨倾斜角(Pitch角)、距骨第一跖骨角(Meary’s角)均较术前改善。根据AOFAS踝与后足功能评分,优13足,良3足,可2足,差0足,优良率为88.89%。结论:运用距下关节制动器Hyprocure治疗儿童柔韧性平足症,操作简单,创伤小,并发症少,中短期效果好。 Objective:To explore the clinical effect of using subtalar joint brake(Hyprocure)in the treatment of children’s flexible flat foot.Methods:From January 2016 to February 2018,12 cases(18 feet)with children’s flexible flat foot were selected,the subtalar arthroereisis was implemented by placing subtalar joint brake(Hyprocure),the changes of the first metatarsal angle(Meary Angle),pitch angle of calcaneus(Pitch Angle),and taloscaphoid coverage angle(TCA)between before and after operation were compared,the efficacy was evaluated by AOFAS ankle and hind foot function score.Results:12 cases were followed up,incision healed in theⅠperiod,patients was wearing shoes,walking,the pain of the affected foot was significantly alleviated compared with preoperative,first metatarsal angle of talus(T1 MT)on weight-bearing orthotopic X-ray.The Pitch angle of calcaneus(Pitch Angle)on the weight-bearing lateral X-ray film and the first metatarsal angle(Meary Angle)were improved compared with preoperative.According to AOFAS ankle and hind foot function score,excellent 13 feet,better 3 feet,good 2 feet,poor 0 feet,excellent and good rate is 88.89%.Conclusion:Subtalar joint brake(Hyprocure)was used to treat flexible flat foot in children,which was simple to operate,small trauma,and fewer complications,and achieved good results in short and medium term.
