玻璃体内注射雷珠单抗治疗肥厚型脉络膜新生血管病变的效果观察

Observation on the efficacy of intravitreal injection of ranibizumab in patients with pachychoroid neovasculopathy

目的观察玻璃体内注射雷珠单抗治疗肥厚型脉络膜新生血管病变(PNV)的临床效果。方法回顾性分析郑州市第二人民医院2017年6月至2019年6月诊治的PNV 30例(30只眼)的临床资料。所有患者接受3次每个月一次的雷珠单抗玻璃体内注射治疗,根据患者病情增加注射次数。随访时间12个月观察注射前后最佳矫正视力(BCVA)、中心区视网膜厚度及脉络膜新生血管面积。结果每位患者平均注射(4.53±1.25)次。术后1、3、6及12个月BCVA较术前提高,差异均有统计学意义(F=2.917,P=0.023)。术后各时间点中央视网膜厚度较术前降低,差异均有统计学意义(F=30.401,P=0.000);术后各时间点脉络膜新生血管面积与术前差异无统计学意义(F=0.150,P=0.963)。结论雷珠单抗玻璃体内注射治疗PNV效果显著。

Objective To observe the clinical efficacy of intravitreal injection of ranibizumab(IVR)in patients with pachychoroid neovasculopathy(PNV).Methods The clinical data of 30 eyes of 30 cases with PNV in Zhengzhou Municipal Second People’s Hospital from Jun.2017 to Jun.2019 were analyzed retrospectively.All patients received 3 rounds of IVR treatment once a month.And the times of injection were increased according to the conditions of patients.The follow-up time was 12 months.The best corrected visual acuity(BCVA),central retinal thickness(CRT)and choroidal neovascularization(CNV)area were observed before and after injection.Results Every patient underwent IVR for(4.53±1.25)times.The BCVA at 1,3,6,and 12 months after injection were ameliorated compared with that before injection(F=2.917,P=0.023),and the differences were statistically significant before and after injection(t=3.365,3.755,3.755,3.755;all P<0.05).There were statistically significant differences in CRT at different time points(F=30.401,P=0.000).The CRT had reduced at 1,3,6,and 12 months after injection,and the differences were statistically significant before and after injection(t=9.651,8.917,9.223,9.260;all P=0.000).There was no significant difference in CNV area before and after injection(F=0.150,P=0.963).Conclusion Intravitreal injection of ranibizumab is effective for the treatment of PNV.
