摘要
目的:分析医联体模式下县域医疗设备临床应用总体效果,为促进医联体内医疗设备资源的配置提供参考依据。方法:基于医务人员真实体验,构建包含6个维度的设备临床应用效果评价指标体系,使用自制问卷调查中西部10个贫困县县域医联体内医务人员设备使用体验。基于奖罚函数计算动态权重,应用物元分析方法进行应用效果评价。结果:90%的县域医联体内医疗设备临床应用效果物元评价等级与人员主观认知相同,但物元分析将医联体间评价差异由9.64%扩大至14.01%;医疗设备临床应用效果评级的平均物元关联度为0.7667,分别在镇巴和大名县域医联体内取得最大和最小值;物元评价等级“良好”的有3个县,“中等”6个县,“较差”1个县。结论:动态物元分析结果克服了主观评价的缺陷,提升了评价结果分辨度;贫困地区县域医联体内基于体验的医疗设备临床应用效果总体一般,且在不同医联体间存在差异。
Objective:To analyze the clinical application effects of medical equipment in county medical treatment alliance,and to provide reference for promoting the optimal allocation of medical equipment resources.Methods:A clinical application experience evaluation index system consisting of six dimensions was constructed based on the real experience of medical staffs.The self-made questionnaire was used to collect the clinical application experience by medical staffs in 10 poverty county medical treatment alliance of central and western China.The weight penalty function was used to calculate the dynamic weight,and the matter-element analysis method based on the extension set theory was applied to evaluate the application effection.Results:90%of the medical entity's matter-element analysis evaluation results and subjective cognitive results were the same,but the difference between the inter-county evaluation results was expanded from 9.64%to 14.01%.The comprehensive correlation degree of equipments'clinical application experience in 10 county medical treatment alliance was 0.7667.From the evaluation level,the evaluation level of material-element analysis for three medical treatment alliance was"good",the"medium"was 6,and one of the"poor".Conclusion:The results of dynamic matter-element analysis making the evaluation results more accurate.The clinical application experience of medical equipment between medical treatment alliance in different counties in poverty-stricken areas is quite different and needs to be further improved.
