全科规培医师在三甲医院风湿免疫科轮转存在的问题及对策

The Problems and Countermeasures of the Rotation of General Practitioners in Department of Rheumatology and Immunology in Third-grade Class-A Hospitals

风湿免疫病为社区常见疾病,关节炎或风湿病为社区老年人最常见的共病之一。风湿免疫科轮转是全科医师规培的重要组成部分,因病种多、发病机制不清、临床表现复杂、多数疾病无法根治等特点,风湿免疫科是整个全科规培中的重点和难点。但目前全科规培医师在风湿免疫科轮转过程中,存在轮转时间少、理论学习不扎实,缺乏门诊常见病种见习计划等诸多问题,一定程度上影响了全科医师今后对社区常见风湿病的诊治能力。如何更好的让全科规培医师在三甲医院风湿免疫科进行培训,是当前规培制度改革亟需解决的问题。文章就这些问题给出建议,希望能为全科医学教育作出有益探索。

Rheumatic disease is a common disease in the community.Arthritis or rheumatism is one of the most common comorbidities of the elderly in the community.The rotation of rheumatology and immunology department is an important part of general practitioner training due to many characteristics such as large catalog,unclear pathogenesis,complex clinical manifestations,and most diseases can not be cured.At present,there are many problems such as less rotation time,lack of solid theoretical learning,lack of internship plan for common diseases in outpatient department which may affects the ability of general practitioners to serve the health of residents in the rotation of rheumatology and immunology department in the standardized training of general practitioners.How to train the general practitioners in rheumatology and immunology department of the third-grade class-A Hospitals is an urgent problem to be solved in the current reform of the training system.This paper gives some suggestions on this issue,hoping to make useful exploration for general practitioner training.
