某院使用PPIs预防围手术期应激性溃疡的分析

The analysis of PPIs prophylactic therapy for stress ulcer of perioperative patients in the hospital

目的:动态观察质子泵抑制剂(PPIs)预防性应用于应激性溃疡的情况。方法:每3个月随机抽取围手术期使用PPI的60份病历,调取患者基本信息和PPI使用情况,对病历进行合理性评价,同时将点评结果反馈给临床科室。结果:240份病历中,Ⅰ类切口使用最多(155例)。其中,186份病历有预防使用的指征。较为常见的应激源/高危因素包括:大手术、高龄、大剂量激素使用史、脑出血等因素。单用奥美拉唑最多(117例)。未见不良反应发生。结论:整体使用指征明确,安全性高,但给药时间、剂型选择和给药疗程方面有待于进一步改进。

Objective:The purpose of this study is to observe prophylactic therapy of proton pump inhibitors(PPIs)to stress ulcers in our hospital dynamically.Methods:Every quarter,60 medical records which used PPI prophylactically in perioperative period were randomly selected,and basic information of patients and PPIs use was collected.Then,these medical records were evaluated rationally,and the results were fed back to the clinic departments.Results:A total of 240 operations were observed with 155 type I incision operations,and 186 medical records had indications for prophylactic therapy.The high risk factors commonly included:major surgery,advanced age,history of large doses of hormones,and cerebral hemorrhage,etc.117 cases applied omeprazole alone.No adverse drug reactions were found.Conclusions:Most 240 medical records had medication indications which was safe.Dosing time,dosage form selection and medicine treatment course need to be improved in the future.
