温和灸配合药物治疗甲状腺功能减退症的临床研究

Clinical Study of Mild Moxibustion Combined with Medication for Hypothyroidism

目的观察温和灸配合药物治疗甲状腺功能减退症的临床疗效。方法将64例甲状腺功能减退症患者随机分为治疗组和对照组,每组32例。治疗组采用温和灸配合口服左甲状腺素钠片治疗,对照组采用单纯口服左甲状腺素钠片治疗。观察两组治疗前后各项血脂指标[血清总胆固醇(TC)、甘油三酯(TG)及低密度脂蛋白胆固醇(LDL-C)]、甲状腺功能指标[促甲状腺激素(TSH)、游离三碘甲状腺原氨酸(FT3)、游离四碘甲状腺原氨酸(FT4)]及血清干扰素-g(IFN-γ)、肿瘤坏死因子-a(TNF-α)水平的变化情况,并比较两组治疗后不良反应发生率。结果两组治疗后各项血脂指标、甲状腺功能指标及血清IFN-γ、TNF-α水平与同组治疗前比较,差异均具有统计学意义(P<0.05)。治疗组治疗后各项血脂指标、甲状腺功能指标及血清IFN-γ、TNF-α水平与对照组比较,差异均具有统计学意义(P<0.05)。治疗组治疗后不良反应发生率为9.4%,对照组为15.6%,两组比较差异无统计学意义(P>0.05)。结论温和灸配合药物治疗甲状腺功能减退症疗效确切,可明显改善患者血脂水平,提高甲状腺功能,这可能与其降低患者血清IFN-γ、TNF-α水平有关。

Objective To observe the clinical efficacy of mild moxibustion plus medication in treating hypothyroidism.Method Sixty-four patients with hypothyroidism were randomized into a treatment group and a control group,with 32 cases in each group.The treatment group was intervened by mild moxibustion plus oral administration of Levothyroxine sodium tablets,while the control group only received oral administration of Levothyroxine sodium tablets.The blood lipid parameters[serum total cholesterol(TC),triglyceride(TG)and low-density lipoprotein cholesterol(LDL-C)],thyroid function indicators[thyroid-stimulating hormone(TSH),free triiodothyronine(FT3)and free tetraiodothyronine(FT4)]and serum interferon(IFN)-g and tumor necrosis factor(TNF)-a were observed before and after treatment in the two groups.The adverse reaction rate was also compared between the two groups after treatment.Result The blood lipid parameters,thyroid function indicators and serum levels of IFN-γand TNF-αchanged significantly after treatment in both groups(P<0.05).After treatment,the blood lipid parameters,thyroid function indicators and serum levels of IFN-γand TNF-αin the treatment group were significantly different from those in the control group(P<0.05).After treatment,the adverse reaction rate was 9.4%in the treatment group versus 15.6%in the control group,and the between-group difference was statistically insignificant(P>0.05).Conclusion Mild moxibustion plus medication is effective in treating hypothyroidism;it can notably improve the blood lipid levels and enhance the thyroid function in the patients,which may be associated with its down-regulation of the serum levels of IFN-γand TNF-α.
