β-hCG、PA、CRP在胎膜早破诊断中的临床价值

Clinical Value of β-hCG,PA and CRP in Diagnosis of Premature Rupture ofMembranes

目的分析人绒毛膜促性腺激素(β-human chorionic gonadotropin,β-hCG)、前清蛋白(prealbumin,PA)、C-反应蛋白(C-reactive protein,CRP)在胎膜早破诊断中的临床价值。方法选取2018年1月至2019年12月东海县人民医院收治的120例单胎分娩足月孕产妇作为研究对象,其中胎膜早破60例作为观察组,正常妊娠60例作为对照组。检测受试者血中β-hCG、PA、CRP水平,并分析各指标单独诊断及联合诊断的诊断价值,采用平行联合和系列联合诊断模式行联合诊断,其中任意指标阳性则平行联合诊断阳性,所有指标均为阳性则系列联合阳性。结果观察组患者血中β-hCG及CRP水平均显著高于对照组,差异有统计学意义(P<0.05),观察组血中PA水平显著低于对照组,差异有统计学意义(P<0.05)。血β-hCG诊断阳性者57例,血PA诊断阳性者57例,CRP诊断阳性者58例,平行联合诊断阳性者76例,系列联合诊断阳性者44例,平行联合诊断灵敏度及阴性预测值显著高于各指标单独诊断,差异有统计学意义(P<0.05),系列联合诊断的特异度及阳性预测值显著高于各指标单独诊断,差异有统计学意义(P<0.05),且平行联合及系列联合诊断的曲线下面积(AUC)显著高于各指标单独诊断,差异有统计学意义(P<0.05)。结论在对胎膜早破进行诊断时采用β-hCG、PA、CRP联合诊断可有效提高诊断效能。

Objective To analyze the clinical value ofβ-hCG、PA、CRP in the diagnosis of premature rupture of membranes.Methods Totally 120 single-birth full-term pregnant women were selected from January 2018 to December 2019 in Donghai County People's Hospital as the study object,60 cases of premature rupture of membranes as the observation group and 60 cases of normal pregnancy as the control group;the bloodβ-hCG、PA、CRP level of the subjects was detected,and the diagnostic value of each index alone diagnosis and combined diagnosis was analyzed.Results The blood levels ofβ-HCG and CRP in the observation group were significantly higher than those in the control group,and the differences were statistically significant(P<0.05).The blood levels of PA in the observation group were significantly lower than those in the control group,and the differences were statistically significant(P<0.05).β-HCG diagnosis was 57 cases of blood,and blood PA diagnosis was 57 cases,58 cases of CRP in the diagnosis of positie,parallel joint diagnosis was 76 cases,a series of joint diagnosis was 44 cases,parallel combined diagnostic sensitivity and negative predictive value is significantly higher than the index diagnosis separately,the difference was statistically significant(P<0.05),a series of specific joint diagnosis and positive predictive value is significantly higher than the index diagnosis separately,the difference was statistically significant(P<0.05),and the parallel series of joint and joint diagnosis of area under curve(AUC)is significantly higher than the index diagnosis alone,The difference was statistically significant(P<0.05).Conclusion The combined diagnosis ofβ-hCG、PA、CRP in the diagnosis of premature rupture of membranes can effectively improve the diagnostic efficacy.
