斑点追踪技术在高血压患者左心房功能早期改变评价中的应用

Application of Speckle Tracking Imaging in Evaluation of Early Changes of Left Atrial Function in Hypertensive Patients

目的探讨斑点追踪技术在高血压患者左心房功能改变中的应用价值。方法选取2017年9月至2019年9月重庆康华众联心血管病医院超声科收治的高血压患者60例作为观察组,观察组中包括左心房正常(观察组1)患者33例,左心房扩大(观察组2)患者27例,另选取同期健康体检者60例作为对照组,三组患者均采用常规超声检查与二维斑点追踪技术进行左心房功能的评估,比较检查结果。结果三组研究对象的左室射血分数(LVEF)比较,差异无统计学意义(P>0.05)。二尖瓣进行血流速度检查(E/A)值、舒张期应变率(mSRs)值及舒张晚期应变率(mSRa)值:观察组2<观察组1<对照组(P<0.05);舒张早期应变率(mSRe)值:观察组2>观察组1>对照组(P<0.05)。结论高血压患者在LVEF尚正常时就已经存在左心房功能的改变,采用二维超声斑点追踪技术能够比较敏感地对高血压患者左心房功能进行评估。

Objective To explore the value of speckle tracking imaging(STI)in the change of left atrial function in patients with hypertension.Methods From September 2017 to September 2019,60 patients with hypertension in Chongqing Kanghua Zhonglian Cardiovascular Hospital were selected as the observation group,including 33 patients with normal left atrium(observation group 1)and 27 patients with enlarged left atrium(observation group 2),and 60 healthy people in the same period were selected as the control group.The three groups were evaluated with conventional ultrasound and two-dimensional STI for left atrial function,and the results were compared.Results There was no significant difference in left ventricular ejection fraction(LVEF)between the three groups(P>0.05).E/A,mSRs and mSRa values:observation group 2<observation group 1<control group(P<0.05);mSRe value:observation group 2>observation group 1>control group(P<0.05).Conclusion Left atrial function has been changed in patients with hypertension when LVEF is still normal.Using two-dimensional ultrasound STI can sensitively evaluate left atrial function in patients with hypertension.
