腹腔镜胆总管囊肿切除联合肝总管空肠Roux-en-Y吻合手术治疗小儿胆总管囊肿的临床效果

Clinical effect of laparoscopic choledochal cyst resection combined with Roux-en-Y hepaticojejunostomy in the treatment of choledochal cyst in children

目的探讨腹腔镜胆总管囊肿切除联合肝总管空肠Roux-en-Y吻合手术治疗小儿胆总管囊肿的临床效果。方法将32例胆总管囊肿患儿采用随机列表法分为对照组和观察组,各16例。对照组接受常规开放胆总管囊肿切除肝总管空肠吻合治疗,观察组接受腹腔镜胆总管囊肿切除联合肝总管空肠Roux-en-Y吻合术治疗。比较两组的手术相关指标、实验室指标。结果观察组的术中出血量少于对照组,手术时间及住院时间均短于对照组(P<0.05)。手术后,观察组的TBIL、ALP、ALT、AST水平均低于对照组(P<0.05)。结论腹腔镜胆总管囊肿切除联合肝总管空肠Roux-en-Y吻合手术治疗小儿胆总管囊肿的临床效果显著,可减少术中出血量,缩短手术时间及住院时间,同时可改善实验室指标水平。

Objective To explore the clinical effect of laparoscopic choledochal cyst resection combined with Roux-en-Y hepaticojejunostomy in the treatment of choledochal cyst in children.Methods A total of 32 children with choledochal cyst were divided into control group and observation group by the random list method,with 16 cases in each group.The control group received conventional open choledochal cyst resection and hepaticojejunostomy,while the observation group received laparoscopic choledochal cyst resection combined with Roux-en-Y hepaticojejunostomy.The related operation indexes,laboratory indexes were compared between the two groups.Results The intraoperative blood loss volume of the observation group was less than that of the control group,and the operation time and hospitalization time were shorter than those of the control group(P<0.05).After operation,TBIL,ALP,ALT and AST levels in the observation group were lower than those in the control group(P<0.05).Conclusion Laparoscopic choledochal cyst resection combined with Roux-en-Y hepaticojejunostomy in the treatment of children with choledochal cyst has significant clinical effect,which can reduce intraoperative blood loss,shorten the operation time and hospitalization time,and improve the level of laboratory indexes.
