全乳晕入路腔镜手术在甲状腺良性疾病中的应用研究

A comparative study on endoscopic thyroidectomy via completely areola approach and chest breast approach

目的比较全乳晕入路与胸乳入路腔镜手术在单侧叶切除术中的应用效果。方法选取青岛市市立医院自2018年10月至2019年12月间收治的95例甲状腺良性疾病行单侧腔镜甲状腺手术患者,按照随机数字表抽取法将其分成全乳晕入路组(观察组)与胸乳入路组(对照组)。观察两组手术时间、术中出血量、术后引流量、术后引流管放置时间、术后胸前不适感发生率、术后并发症发生率及患者满意度等情况。结果观察组和对照组患者的手术时间、术中出血量、术后拔管时间、暂时性甲状旁腺功能减退率、术后出血发生率、术后喉返神经损伤发生率、喉上神经损伤发生率比较,差异均无显著性,但术后引流量、术后胸前血清肿发生率、术后胸部不适感发生率、术后3个月切口美容效果满意度差异均有显著性。结论全乳晕入路腔镜甲状腺手术治疗效果显著,具有微创、并发症少、切口美观的优点。

Objective To compare the effect of complete areola approach and chest breast approach endoscopic thyroidectomy in thyroid unilateral lobectomy.Methods Ninety-five cases with benign thyroid disease were selected to operate undergoing endoscopes unilateral lobectomy of the thyroid endoscopic operation from October 2018 to December 2019 in our hospital.According to the random number table extraction method,they were divided into complete areolar approach group(observation group)and chest breast approach group(control group).Observe the two groups in operative time,intraoperative blood loss,postoperative flow and postoperative drainage tube placement time,the incidence of postoperative chest discomfort,postoperative complications and patient satisfaction.Results Observation group and control group in patients with operative time,intraoperative blood loss,postoperative extubation time,temporary parathyroid function decline rate,incidence of postoperative bleeding,the incidence of postoperative laryngeal recurrent nerve injury,the incidence of laryngeal nerve injury on the comparison,there were no statistically significant difference,but the rate of postoperative drainage,postoperative thoracic serum swelling,postoperative chest discomfort,and postoperative incision beauty effect were statistically significant.Conclusion The effect of total areola in the treatment of endoscopic thyroidectomy is remarkable,with the advantages of minimally invasive,less complication and beautiful incision.
