血清巨噬细胞移动抑制因子、可溶型细胞间黏附分子-1及甲胎蛋白水平与慢性乙型肝炎患者肝纤维指标的关联性分析

The correlation between serum MIF,sICAM-1,AFP levels and liver fiber indexes in patients with chronic hepatitis B

目的探讨慢性乙型肝炎患者血清巨噬细胞移动抑制因子(MIF)、可溶型细胞间黏附分子-1(sICAM-1)及甲胎蛋白(AFP)水平与肝纤维化指标关联性。方法我院收治的110例慢性乙肝肝炎患者(研究组)及同期104例健康体检者(对照组),两组均测定血清MIF、sICAM-1、AFP水平及透明质酸(HA)、层黏蛋白(LN)、Ⅳ型前胶原(Ⅳ-C)水平,评估研究组肝纤维化程度。比较两组血清三指标水平及肝纤维化指标水平,比较不同肝纤维化程度患者血清三指标水平,分析血清三指标水平与肝纤维化指标及纤维化程度的相关性,分析血清三指标水平对患者肝纤维化预测价值。结果研究组血清MIF、sICAM-1、AFP水平及HA、LN、Ⅳ-C水平高于对照组(P<0.05);随着肝纤维化程度加重,血清MIF、sICAM-1及AFP水平上升(P<0.05);血清MIF、sICAM-1、AFP水平与HA、LN、Ⅳ-C、肝纤维化程度正相关(P<0.05);血清MIF、sICAM-1、AFP水平预测慢性乙型肝炎患者肝纤维化的AUC分别为0.792、0.806、0.813。结论慢性乙型肝炎患者血清MIF、sICAM-1及AFP水平与患者肝纤维指标关系密切,可用于预测患者肝纤维化情况。

Objective To explore the correlation between serum macrophage migration inhibitory factor(MIF),soluble intercellular adhesion molecule-1(sICAM-1),alpha-fetoprotein(AFP)levels and liver fibrosis indexes in patients with chronic hepatitis B(CHB).Methods One hundred and ten CHB patients(study group)who were admitted to our hospital and 104 healthy people(control group)who underwent physical examination were enrolled.The levels of serum MIF,sICAM-1 and AFP,as well as hyaluronic acid(HA),laminin(LN),type-IV procollagen(IV-C)in both groups were measured.The degree of liver fibrosis in the study group was evaluated.The levels of the three serum indexes and liver fibrosis indexes were compared between the two groups.The levels of the three serum indexes were compared among patients with different degrees of liver fibrosis.The correlation between the three serum indexes and liver fibrosis indexes and degree was analyzed.The predictive value of the three serum indexes for liver fibrosis was analyzed.Results The levels of serum MIF,sICAM-1 and AFP,as well as HA,LN,IV-C in the study group were significantly higher than those in the control group(P<0.05).With the exacerbation of liver fibrosis,levels of serum MIF,SICAM-1 and AFP were increased gradually(P<0.05).The results of Pearson correlation analysis showed that levels of serum MIF,sICAM-1 and AFP were positively correlated with HA,LN,IV-C and liver fibrosis degree(P<0.05).ROC curve analysis showed that AUC values of serum MIF,sICAM-1 and AFP levels for predicting liver fibrosis in CHB patients were 0.792,0.806 and 0.813,respectively.Conclusion The levels of serum MIF,sICAM-1 and AFP are closely related to liver fiber indexes in CHB patients,which can be applied to predict the liver fibrosis status.
