地特胰岛素联合阿卡波糖治疗老年2型糖尿病的临床疗效

Clinical effect of insulin detemir combined with acarbose in the treatment of elderly patients with type 2 diabetes

目的分析地特胰岛素联合阿卡波糖治疗老年2型糖尿病的效果。方法选取天津市西青区辛口镇社区卫生服务中心2017年1月~2019年5月老年2型糖尿病患者182例,随机数字表法分为两组,其中试验组91例给予地特胰岛素联合阿卡波糖治疗,对照组91例给予地特胰岛素联合格列美脲治疗,比较两组治疗不同时期血糖水平[空腹血糖(FBG)、餐后2 h血糖(2 hPG)]、血糖波动情况(平均血糖、最低血糖、最高血糖、血糖波动水平)、不良事件(低血糖、体重增加)情况。结果治疗后1个月、2个月、3个月,两组FBG、2 hPG水平低于治疗前(P<0.05);试验组最低血糖高于对照组,血糖波动水平低于对照组(P<0.05);试验组低血糖发生率1.10%(1/91),低于对照组10.99%(10/91)(P<0.05);试验组增加体重少于对照组(P<0.05)。结论地特胰岛素联合阿卡波糖治疗老年2型糖尿病患者,可有效控制血糖,降低低血糖风险,维持血糖稳定,且对体重影响较小。

Objective To analyze the effect of insulin detemir combined with acarbose in the treatment of elderly patients with type 2 diabetes.Methods A total of 182 elderly patients with type 2 diabetes in Community health service center of Xinkou town from January 2017 to May 2019 were selected and divided into 2 groups by random number table method.Both groups were given insulin detemir.Among them,91 cases were combined with acarbose as the test group,and the other 91 cases were combined with glimepiride as the control group.Comparing the blood glucose levels[fasting blood glucose(FBG),2 h postprandial blood glucose(2 hPG)],and blood glucose fluctuations(mean blood glucose,minimum blood glucose,maximum blood glucose,blood glucose fluctuation levels)and adverse events(hypoglycemia,body mass increase)of the two groups during treatment.Results The levels of FBG and 2 hPG in 2 groups at 1 month,2 months and 3 months after treatment were lower than before treatment(P<0.05);the lowest blood glucose in the test group was higher than that in the control group,and the blood glucose fluctuation level was lower than that in the control group(P<0.05);the incidence of hypoglycemia in the test group was 1.10%(1/91)lower than that in the control group,10.99%(10/91)(P<0.05);the increase in body weight of the test group was less than that of the control group(P<0.05).Conclusion Insulin detemir combined with acarbose in the treatment of elderly patients with type 2 diabetes can effectively lower blood sugar,reduce the risk of hypoglycemia,maintain blood sugar stability,and have little effect on body weight.
