阿司匹林联合低分子肝素对血栓前状态致复发性流产的临床疗效和安全性研究

Clinical efficacy and safety of aspirin combined with low molecular weight heparin in recurrent spontaneous abortion caused by prethrombotic state

目的研究阿司匹林联合低分子肝素对血栓前状态(PTS)所致复发性流产(RSA)患者的临床疗效和安全性。方法采用整群抽样法抽选2019年1月~2019年12月收治的PTS所致RSA共68例患者进行研究,依据随机数表法将其中34例纳入对照组(予以阿司匹林),余下34例纳入观察组(于对照组基础上加以低分子肝素),比较两组妊娠结局、治疗前后凝血酶时间(TT)、D-二聚体(D-D)、β-人绒毛膜促性腺激素(β-HCG)水平和不良反应发生情况。结果观察组活产率比对照组更高,流产率比对照组更低,差异有统计学意义(P<0.05)。治疗前,两组TT、D-D及β-HCG水平差异无统计学意义(P>0.05);治疗后,观察组的TT及β-HCG水平比对照组更高,D-D水平比对照组更低,差异均有统计学意义(P<0.05)。两组不良反应差异无统计学意义(P>0.05)。结论阿司匹林联合低分子肝素治疗PTS所致RSA的疗效显著,且安全性较高。

Objective To study the clinical efficacy and safety of aspirin combined with low molecular weight heparin in the treatment of recurrent spontaneous abortion(RSA)caused by prethrombotic state(PTS).Methods A total of 68 patients with RSA caused by PTS from January 2019 to December 2019 were selected by cluster sampling.34 of them were included in the control group(aspirin)and the remaining 34 in the observation group(low molecular weight heparin on the basis of the control group).The pregnancy outcome,thrombin time(TT),Dimer(D-D),β-human chorionic gonadotropin(β-HCG)levels and adverse reactions of the two groups were compared.Results The live birth rate of the observation group was significantly higher than that of the control group,and the abortion rate was significantly lower than that of the control group(P<0.05).Before treatment,there was no significant difference in TT,D-D and β-HCG levels between the two groups(P>0.05);after treatment,TT and β-HCG levels in the observation group were significantly higher than those in the control group,and D-D levels were significantly lower than those in the control group(P<0.05).There was no significant difference in adverse reactions between the two groups(P>0.05).Conclusion Aspirin combined with low molecular weight heparin is effective and safe in the treatment of RSA caused by PTS.
