半夏泻心汤化裁联合兰索拉唑三联疗法治疗Hp相关消化性溃疡

Banxia Xiexin Decoction Combined with Lansoprazole in the Treatment of Hp-Associated Peptic Ulcer

目的:观察半夏泻心汤化裁联合兰索拉唑三联疗法治疗幽门杆菌(Hp)相关消化性溃疡的疗效。方法:选取2017年5月至2018年5月大连市中医医院收治的Hp阳性消化性溃疡患者90例作为研究对象,按照随机数字表法随机分为对照组和观察组,每组45例。对照组患者给予兰索拉唑三联疗法,观察组患者则在对照组基础上加用半夏泻心汤化裁方,7 d为1个疗程,连续用药4个疗程。比较2组治疗方案的有效率、Hp根除率、胃肠激素、炎性因子、用药安全性、复发率。结果:与对照组73.33%的有效率比较,观察组的有效率高达91.11%,升高明显(P<0.05);观察组、对照组的Hp根除率分别为77.78%、44.44%,以观察组根除Hp效果更好(P<0.05);观察组患者治疗后的胃肠激素SS水平升高明显,MTL、GAS水平降低明显(P<0.05);血清中TNF-α、IL-6水平降低明显,TGF-β水平升高明显(P<0.05);对照组不良反应发生率20.00%、复发率27.78%明显高于观察组的不良反应发生率6.67%、复发率3.70%(P<0.05)。结论:半夏泻心汤化裁方联合兰索拉唑三联疗法治疗Hp阳性消化性溃疡疗效肯定,利于根除Hp,用药安全可靠,不易于复发,其机制可能与调控胃肠激素水平、抑制炎性因子表达有一定相关性。

Objective:To observe the efficacy of Banxia Xiexin Decoction Huazai Formula combined with triple therapy of lansoprazole in the treatment of Hp-associated peptic ulcer.Methods:A total of 90 patients with Hp-positive peptic ulcer in Dalian Traditional Chinese Medicine Hospital were divided into an observation group and a control group according to the random number table,with 45 cases in each group.Patients in the control group were given the triple therapy of lansoprazole,while patients in the observation group were treated with Banxia Xiexin Decoction Huazai Formula on the basis of the control group.A course of treatment lasted for 7 days,with 4 consecutive courses of medication.The effective rate,Hp eradication rate,gastrointestinal hormones,inflammatory factors,medication safety and recurrence rate of the 2 groups were compared.Results:Compared with the effective rate of 73.33%in the control group,the effective rate of 91.11%in the observation group was significantly increased(P<0.05).The eradication rates of Hp in the observation group and the control group were 77.78%and 44.44%respectively,and the eradication effects of Hp in the observation group was better(P<0.05).After treatment,patients in the observation group showed significant increase in gastrointestinal hormone SS level and significant decrease in MTL and GAS levels(P<0.05).Serum levels of TNF-αand IL-6 were significantly decreased and TGF-βwere significantly increased(P<0.05).The incidence of adverse reactions in the control group was 20.00%and the recurrence rate was 27.78%,which were significantly higher than that in the observation group of 6.67%and 3.70%(P<0.05).Conclusion:Pinxia Xiexin Decoction Huazai Formula combined with triple therapy of lansoprazole in the treatment of Hp-associated peptic ulcer has a positive curative effects,which is conducive to the eradication of Hp,with safe and reliable drug use,and it is not easy to relapse.The mechanism may be related to the regulation of gastrointestinal hormone levels and the inhibition of inflammatory factor expression.
